L’évolution récente du système de santé et de services sociaux : étapes et enjeux

Following the recommendations of the Commission of study on health services and social services (2000) (The Clair Commission), as well as much reflection and several consultations aimed at adapting Quebec’s health and social services system to the challenges confronting it, many legislative and administrative changes have been introduced in recent years which have substantially modified the organization and governance of services. Measures have been implemented to promote better access to services and a better integration of services into functional networks, precisions have been made concerning the responsibilities of the various bodies and means have been put in place to make organizations more concretely accountable through a more results-based approach to management. In 2006, legislative amendments have allowed for a mechanism to be put in place with regard to improving the management of access to specialized services in the public system and guidelines have been introduced to better manage the supply of services in the private sector. More recently, the report of the Task Force on the Funding of the Health System (2008) (the Castonguay Report) prompted the health ministry and the Government of Quebec to come up with some structuring means of increasing the productivity and performance of the system. Despite these changes, the recurring issues pose a major challenge to the long-term viability of the system.Dans la foulée des recommandations du rapport de la Commission d’étude sur les services de santé et les services sociaux (2000) (Commission Clair) et de divers questionnements et consultations visant à adapter le système sociosanitaire québécois aux défis qui le confrontent, les dernières années ont été marquées par l’introduction d’ajustements législatifs et administratifs qui modifient substantiellement l’organisation des services et leur gouvernance. Des mesures ont été mises en oeuvre pour favoriser un meilleur accès aux services et leur intégration dans des réseaux fonctionnels, des précisions ont été apportées aux responsabilités des différentes instances et des moyens ont été mis en place pour concrétiser l’exercice de l’imputabilité des organisations dans une gestion davantage axée sur les résultats. En 2006, des modifications législatives ont permis l’implantation d’un mécanisme de gestion de l’accès aux services spécialisés dans le système public ainsi que l’introduction de balises pour encadrer l’offre du secteur privé. Plus récemment, le rapport du Groupe de travail sur le financement du système de santé (2008) (Rapport Castonguay) incite le ministère de la Santé et des Services sociaux et le gouvernement québécois à poser des gestes structurants pour accroître la productivité et la performance. Malgré ces changements, la récurrence des enjeux pose un défi majeur pour la pérennité du système.Quenneville Andrée, Clavet Michel, Dubé François.
