Tensions et orientations du système français de santé au travail

The French occupational health system was established on the basis of views which hold two particular characteristics: an approach to work-related health problems based much more on compensation than on prevention, and pre-eminence given to the medical approach, mainly centred on individual health and selecting population groups. From 1946 onwards, these traditions became regulations thus constituting an obstacle to the evolution of the doctrine and a factor which paralyzed a health system mainly oriented towards determining the aptitude of individuals. It was not until the 1970s that the need to take action on working conditions emerged. This movement gathered speed as of the late 1980s, under the influence of the harmonization of European legislation, which compelled France to catch up, and under the pressure of asbestos victims associations. The adoption in 2005 of the Plan Santé Travail (occupational health plan) by the government attests to this need for a change in policy. The situation is nevertheless marked by a worrying factor, that is, French employees’ capacity to voice their opinions has weakened, despite the fact that a genuine policy on prevention cannot be considered without their participation.Les conceptions qui ont conduit à la mise en place du système français de santé au travail présentent deux caractéristiques particulières: une approche des atteintes à la santé par le travail en terme d’indemnisation beaucoup plus qu’en terme de prévention et une prééminence de l’approche médicale, essentiellement préoccupée d’hygiène des individus et de tri des populations. À partir de 1946, la fixation de ces traditions dans les dispositifs réglementaires va constituer un obstacle à l’évolution de la doctrine et un facteur de paralysie d’un système de santé essentiellement orienté vers la détermination de l’aptitude des individus. Il faudra attendre les années 1970 pour voir émerger l’exigence d’une action sur les conditions de travail. Ce mouvement a connu une très sérieuse accélération à partir de la fin des années 1980, sous l’influence de l’harmonisation de la législation européenne, qui contraint la France à rattraper son retard, et sous la poussée des associations de victimes de l’amiante. L’adoption du Plan Santé Travail par le gouvernement, en 2005, témoigne de cette exigence de réorientation. La situation demeure néanmoins marquée par un facteur préoccupant: l’affaiblissement des capacités d’expression des salariés français, alors qu’une réelle politique de prévention est difficilement imaginable sans leur participation.Davezies Philippe.
