The blood-brain barrier is extremely selective to protect the brain from xenobiotics in systemic circulation. These physiological properties are linked to the cerebral microvascular endothelial cells. This brain protection is a major drawback to treat cerebral pathologies with biotherapeutics and specifically with antibodies. The best current strategy to cross the blood-brain barrier with antibodies is the "Trojan horse" strategy, making use of receptors mediating transcytosis across the blood-brain barrier.
