Erythropoietin Produktion bei akuter und chronischer obstruktiven Nephropathie

EPO is a circulating hormon that stimulates erythrocyte generation in bone marrow. EPO is secreted by the kidney and its production is stimulated following hypoxic stimuli. Little is known about the influence of obtructive nephropathy on hypoxia-stimulated EPO production. Therefore, we established a model of obstructive nephropathy (ONP) by unilateral ureteral ligation (UUL) in rats which were subjected to CO and hypobaric hypoxia (8% O2). We also tested the reversibility of UUL on EPO plasma levels by stenting the ligated ureter segment to allow re-establishment of urine flow. To assess the renal EPO secretory capacity of UUL kidneys we performed a contralateral nephrectomy. The results show that EPO plasma levels were elevated by both hypoxic stimuli 50 to 300 fold in controls. In contrast EPO secretion of UUL was greatly reduced. The change in mRNA of EPO paralelled the increase of EPO plasma levels. Re-established urine flow for 72 hours resulted in a 50% recovery of EPO production. In an attempt to analyse the mechanism of the greatly reduced EPO secretory capacity of UUL we measured the tissue content of S-adenosylhomocystein (SAH) of UUL kidneys. We found a dramatic increase in SAH tissue levels and a strong reduction of the tissue methylation potential. Effective EPO gene transcription requires an undisturbed methylation of the 5’ end of EPO mRNA for cap formation. The recovery of EPO secretion following re-established urine flow was associated with a nearly complete normalisation of SAH tissue content. Since it was shown in previous studies from our laboratory that high SAH levels impair EPO mRNA cap formation, reduced EPO gene transcription and mRNA translation we suggest from our results that reduced EPO secretory capacity following hypoxic stimuli in UUL is mediated by elevated SAH tissue levels.Erythropoetin (EPO) ist ein Glykoprotein, das hämatopoetischer Wachstumsfaktor stimulierend auf die Erythropoese wirkt. EPO wird von der Niere freigesetzt. Trigger für die Steigerung der EPO-Produktion ist eine Gewebshypoxie. Der Einfluss der obstruktiven Nephropathie auf Hypoxie stimulierte EPO Produktion ist schon wenig untersucht. Als Modell für die obstruktive Nephropathie wurde Unilaterale Ureterligatur (UUL) bei Ratten, die mit Kholenmonoxide (CO) and hypobaric hypoxie (8% O2) exponiert, durchgeführt. Die Reversebilität von UUl auf EPO Produktion wurde bei Rekanülierung des ligierten Ureters durch Schienung des Ureters mittels eines kleinen Plastikschlauches durchgeführt. EPO Plasma Levels sind bei Kontollgruppen nach den beiden Hypoxiearten um 50 bis 300 Mal angestiegen. Umgekhert ist EPO Produktion bei UUL Gruppen erheblich abgefallen. Veränderung an EPO-mRNA war parallel zu EPO Plasma Levels. 72 Stunden nach Rekanülierung des Liegerten Ureters hat sich EPO Produktion bei 50% erholt. In einem Versuch um den Mechanismus der erheblichen reduzierten EPO-Sekretion zu erklären wurde S-adenosylhomocystein (SAH) der UUL-Nieren bestimmt. Die Erholung der EPO-sekretion nach Rekanülierung des Liegerten Ureters assozieret mit fast komplett SAH Normalisierung der UUL-Nieren. Effektiv EPO-Gen transkription fordert eine normale methylierung des 5’ Endes des EPO-mRNAs für cap formation .Ergebnisse aus unserem Labor haben schon gezeigt, dass ein erhöhtes SAH Level die EPO-mRNA cap formation beeinträchtigt, EPO-gene transkription and mRNA translation reduziert. Wir schlagen angesichts unserer Ergebnisse vor, dass die Reduktion an hypoxie stimulierete bei UUL wird von erhöhten SAH tissue levels vermittelt
