The expression of cadherin 11, the factor VII activating protease, the estrogen and the progesterone receptor and the prognostic meaning in endometrial carcinoma

Beim Endometriumkarzinom stellt sich die Frage ob ein fortgeschrittenes Tumorstadium (FIGO-Stadium ≥Ic) Folge einer verspäteten Diagnose oder aber einer aggressiveren Tumorbiologie ist. In dieser Hinsicht sollte die Expression von Cadherin 11, der Faktor VII aktivierenden Protease (FSAP), des Östrogenrezeptors und des Progesteronrezeptors sowie des Tumorgradings, des Tumorstadiums, der Tumorhistologie, der Lymphangiosis und Hämangiosis carcinomatosa, der myometranen Tumorinvasionstiefe, der Tumornekrose und das Lebensalters bei Diagnosestellung auf eine mögliche differenzierende und/oder prognostische Bedeutung hin untersucht werden. Grundlage der Untersuchung sind die Krankenakten und die in Formalin fixierten und in Paraffin eingebetteten Gewebeschnitte von 183 Patientinnen, die in der Klinik für Frauenheilkunde und Geburtshilfe der Justus-Liebig Universität Giessen behandelt wurden. Statistische Methoden waren die bivariate Korrelation sowie die schrittweise Diskriminanzanalyse. Aufgrund einer nur sehr geringen Rate an tumorassoziierten Todesfällen im vorliegenden Kollektiv, wurde auf die Berechnung der Kaplan-Meier-Überlebenszeitanalyse aufgrund einer geringen Aussagekraft verzichtet. Das Tumorstadium bestätigte sich als der wichtigste prognostische Faktor. Hinsichtlich des tumorassoziierten Todes konnte das Tumorstadium als bedeutendster Parameter gefolgt von der Hämangiosis und der Lymphangiosis carcinomatosa identifiziert werden. Im Hinblick auf eine schnelle Tumorprogression (aggressivere Tumorbiologie) konnte neben der Lymphangiosis carcinomatosa als wichtigstem Parameter, das Tumorgrading gefolgt von der Tumornekrose als beeinflussende Faktoren identifiziert werden. Dies unterstreicht die Wichtigkeit der Beurteilung dieser Parameter im Rahmen der pathologischen Begutachtung auch im Hinblick auf eine darauf basierende risiko-adaptierte Therapie der Patientinnen. Weder der Östrogen- noch der Progesteronrezeptor hatten in diesem Kollektiv eine prognostische Bedeutung. Neben einer Beteiligung von Cadherin 11 an der myometranen Invasion der Tumorzellen deuten die Ergebnisse der bivariaten Analyse auf eine Verbindung von Cadherin 11 mit dem Östrogenrezeptor hin, was auf einer Regulation der beiden Parameter durch Wachstumshormone oder aber auch auf gemeinsam benutzten Signaltransduktionsmechanismen wie dem MAPK- signaling pathway beruhen könnte. Die FSAP bietet keine Korrelation mit anderen prognostischen Parametern und scheint für die Prognose des Endometriumkarzinoms in diesem Kollektiv keine Rolle zu spielen.In endometrial cancer the question arises whether an advanced tumor stage (FIGO stage ≥Ic) consequence of a belated diagnosis or however a more aggressive tumor biology. In this regard should the expressions of cadherin 11, the factor VII activating protease (FSAP), the estrogen receptor and the progesterone receptor as well as the tumor grading, the tumor stage, the histologic type, the lymphangiosis and hemangi-osis carcinomatosa, the depth of myometrial invasion, the tumor necrosis and the age be examined on a possible differentiating and/or prognostic meaning. Basis of the investigation are the medical records and the formalin fixed and in paraffin embedded tissue sections of 183 female patients, who were treated at the department of obstetrics and gynecology of the Justus-Liebig University of Giessen. Statistical methods were the bivariate correlation as well as the stepwise discriminant analysis. Based on an only very slight rate at tumor-associated deaths in the existing collective, was forgone the calculation of the Kaplan-Meier over-lifetime analysis based on a slight significance. The tumor stage was confirmed as the most important prognostic factor. With respect to the tumor-associated death, the tumor stage could be identified as the most important parameter followed of the hemangiosis and the lymphangiosis carcinomatosa. With regard to a fast tumor progression (more aggressive tumor biology) the tumor grading and the tumor necrosis can be identified next to the lymphangiosis carcinomatosa as the most important factor as influencing factors. This underlines the importance of the evaluation of these parameters in the context of the pathological investigation also regarding a risk-adapted therapy of the female patients which is based on it. Neither the estrogen receptor nor the progesterone receptor had a prognostic meaning in this collective. Next to a participation of cadherin 11 at the myometrial invasion of the tumor cells, the results of the bivariate analysis point on a connection of cadherin 11 with the estrogen receptor, what could based upon a regulation both parameter through growth hormones or however also in together used signal transduction mechanisms like the MAPK-signaling pathway. The FSAP offers no correlation with other prognostic parameters and seems to play no role for the prognosis of endometrial cancer in this collective
