Ultrasonography of the fetal nose, maxilla, mandible and forehead as markers for aneuploidy

Tegenwoordig wordt de ‘combinatietest’ in het eerste trimester aan alle zwangeren aangeboden als screeningsmethode voor onder andere het downsyndroom. Echter maakt maar ongeveer 30% van de Nederlandse zwangere vrouwen hier gebruik van. Dit betekent dat wanneer zwangere vrouwen zich melden voor de routine 20-wekenecho in het tweede trimester, relatief veel foetussen een onontdekte chromosomale afwijking hebben. Daarom is het voor de echoscopist van belang om aanvullende metingen te kunnen doen in het tweede en derde trimester, die het vermoeden op een chromosomale afwijking kunnen bevestigen of ontkrachten. In dit proefschrift worden anatomische structuren in het profiel van het foetale aangezicht – het voorhoofd, de neus, onder- en bovenkaak – beschreven die, indien ze sterk afwijken van het gemiddelde, er op kunnen duiden dat de foetus het down- of edwardssyndroom heeft. In een groep foetussen met het downsyndroom bleek retrospectief gemeten dat 86% een sterk afwijkende PT-NBL-ratio (verhouding tussen het neusbotje en de huidplooi voor de neus) heeft, en zelfs 95% een afwijkende PT-NBL-ratio en/of PFSR (verhouding tussen de kaken en huidplooi voor de neus). In een groep foetussen met het edwardssyndroom had 88% een sterk afwijkende PT-NBL-ratio. Meting van de zogenaamde foetale profiellijn, die in het foetale aangezicht de stand tussen de onderkaak en het voorhoofd objectiveert, kan juist mogelijk een onderscheid maken tussen foetussen met het down- of edwardssyndroom. De conclusie van dit proefschrift is dat door middel van simpele metingen in het foetale aangezicht tijdens de 20-wekenecho, de inschatting of de foetus het down- of edwardssyndroom heeft sterk kan worden verbeterd.Nowadays, the ‘combined test’ is offered to all pregnant women as a screening method for, among other things, Down syndrome. However, only an estimated 30% of Dutch women actually apply for this test. This means that when pregnant woman present themselves for the routine 20-weeks scan in the second trimester, a relatively large amount of fetuses have an undetected chromosomal anomaly. Tools and measurements that can help the ultrasonographer with affirming or weakening the suspicion of a chromosomal anomaly, are of great importance. In this thesis, several anatomical features in the fetal profile – the forehead, nose, maxilla and mandible – are described that can raise the suspicion of Down and Edwards syndrome, in case they are strongly abnormal. After the group of fetuses with Down syndrome was measured retrospectively, 86% had a strongly abnormal ‘PT-NBL ratio’ (relation between the nasal bone and the skin anterior of the nose) and even 95% had an abnormal PT-NBL ratio and/or ‘PFSR’ (relation between the jaws and skin anterior of the nose). In the group of fetuses with Edwards syndrome, 88% had an abnormal PT-NBL ratio. Measurement of the ‘fetal profile line’, which assesses the position of the mandible opposed to the forehead, can actually make a potential differentiation in the suspicion of Down or Edwards syndrome. The conclusion of this thesis is that through relatively simple measurements in the fetal profile during the 20-weeks scan, the suspicion of a chromosomal anomaly like Down and Edwards syndrome can be substantiated
