In this case report, we describe a giant biliary tumor of 20 cm in diameter in the right robe of the liver of a 21-year-old male. The distinctive pathological diagnosis of 'biliary adenofibroma with malignant epithelial transformation' and the radiological work-up are discussed, and the literature on giant hepatic mass lesions is reviewed
