Orthophonistes utilisant l'Evidence-Based Practice : profils de cliniciennes et appropriation d'une pratique émergente

Evidence-Based Practice (EBP) is a practice that stems from Evidence Based Medicine and is used in the health field. In France, this approach has been established in the clinical field of competence of speech and language therapists for about ten years. The contribution of the sociology of professions to this practice highlights the issues at stake with regard to professional rhetoric and the process of professionalization of speech therapy.In light of these issues, we ask ourselves what are the characteristics of SLPs practicing EBP in France today and how they have appropriated this practice. To answer this question, we conducted nine semi-structured interviews with nine speech-language pathologists practicing in metropolitan France who were involved in EBP.The analysis of these interviews shows that these speech therapists are attracted to training and passing on their knowledge to their peers through initial and continuing education. In this way, these speech therapists participate in the transmission of knowledge and EBP principles and contribute fully to the professionalization of the profession. They also contribute to bringing the fields of research and clinical practice closer together by investing in these courses and, for some of them, by contributing to research and supervising end-of-studies theses. The desire to practice EBP is rooted in rigorous profiles and a quest for legitimacy consistent with the place of EBP in professional speech-language pathology rhetoric. Nevertheless, although they practice EBP, these speech-language pathologists are also subject to the obstacles commonly attributed to it, notably its time-consuming aspect and the inadequacy of the results of the research to the clinic. These professionals are therefore adapting their EBP, both in the way they consult the literature and in the way they construct measures of effectiveness, so that it is better aligned with their clinic,by adjusting their practice and passing on their knowledge, these professionals are helping to make the EBP more accessible and are thus taking part in the professionalization of speech-languagepathology.L’Evidence-Based Practice (EBP) est une pratique issue de l’Evidence Based Medecine et utilisée dans le domaine de la santé. En France, cette démarche s’installe depuis une dizaine d’années dans le champ de compétences cliniques de l’orthophoniste. L’apport de la sociologie des professions sur cette pratique met en évidence ses enjeux vis-à-vis de la rhétorique professionnelle et du processus de professionnalisation de l’orthophonie.À la lumière de ces enjeux, nous nous demandons quelles sont les caractéristiques des orthophonistes pratiquant l’EBP en France aujourd’hui et comment ils se sont appropriés cette pratique. Pour répondre à cette question, nous avons réalisé neuf entretiens semi dirigés auprès de neuf orthophonistes exerçant en France métropolitaine et inscrivant leur pratique dans une démarche EBP.L’analyse de ces entretiens montre chez ces orthophonistes, un attrait pour se former et transmettre à leurs pairs via la formation initiale et continue. Ces orthophonistes participent de cette manière à transmettre des connaissances ainsi que les principes de l’EBP et contribuent pleinement à la professionnalisation du métier. Elles participent également à rapprocher les domaines de la recherche et de la clinique en s’investissant dans ces formations et pour certaines d’entre elles, en contribuant à la recherche et à l’encadrement de mémoires de fin d’études.À l’origine de la volonté de pratiquer l’EBP, il existe des profils rigoureux ainsi qu’une certaine quête de légitimité en cohérence avec la place que tient l’EBP dans la rhétorique professionnelle orthophonique. Néanmoins, bien que pratiquant l’EBP, ces orthophonistes sont également soumises aux obstacles qui lui sont communément attribués, notamment à son aspect chronophage et à l’inadéquation des résultats de la recherche à la clinique. Ces professionnelles adaptent donc leur EBP, tant part leur manière de consulter les données de la littérature, que par leur manière de constituer des mesures d’efficacité, afin qu’elle soit en meilleure adéquation avec leur clinique.En ajustant leur pratique et en transmettant leurs connaissances, ces professionnelles participent à rendre l’EBP plus accessible et prennent ainsi part à la professionnalisation de l’orthophonie
