Evaluation of the decrease in infections in anterior cruciate ligament reconstruction using vancomycin-immersed grafts: a systematic review

La cirugía de reconstrucción de ligamento cruzado anterior es uno de los procedimientos de rodillas más prevalentes a nivel mundial con una tasa de 45,1 reconstrucciones realizadas por cada 100.000 habitantes. La infección tras la reconstrucción artroscópica de ligamento cruzado anterior (LCA) es una complicación posoperatoria poco frecuente pero potencialmente devastadora con prevalencias reportadas en la literatura que varían entre un 0,14 – 1,7% en las diferentes series de casos (3,6). Existen diferentes vías de colonización e infección de los injertos, sin embargo, se considera que en la gran mayoría puede provenir del propio procedimiento de recolección del injerto o durante la preparación del injerto en la mesa trasera. Parece no existir una diferencia en las tasas de infección cuando se usa antibiótico preoperatorio, versus cuando no se usa, en este tipo de cirugías. Actualmente en todos los estudios reportados en la literatura se usa rutinariamente antibiótico preoperatorio, en la gran mayoría de los casos con la administración de cefalosporinas de primera generación, o el uso de clindamicina o vancomicina en caso de presentarse alergias. El uso de la profilaxis antibiótica local sumergiendo los injertos en vancomicina ha tomado popularidad en los últimos años.
