Contribuições da enfermagem obstétrica para a humanização do parto : um olhar sobre o (des)uso da episiotomia

Objective: To analyze nursing production on the (dis)use of episiotomy in the context of childbirth care. Methodology: Integrative literature review, carried out in October 2022, based on the analysis of articles available in full between the years 2017 to 2021, through the Virtual Health Library (BVS), in the Medical Literature Analysis and Retrieval databases System Online (MEDLINE), Latin American and Caribbean Literature in Health Sciences (LILACS), Database in Nursing (BDENF), as well as in the Scientific Electronic Library Online (SciELO). Results: Among the indications for performing the procedure, the following stand out: dystocia, fetal distress, macrosomia, prolonged expulsive period and operative delivery. There is a reaffirmation regarding the non-recommendation of the procedure for not presenting sufficient and effective scientific evidence to support its use. Regarding the consequences, the most cited are: hemorrhage, dyspareunia, infection, urinary and fecal incontinence, pain and sexual dysfunction. It was noted the defense of the humanization of childbirth through transparent and effective care that comes through the academic training of professionals, especially nursing, in which it is responsible for welcoming, supporting and guiding women throughout the childbirth process. Conclusion: the practice of episiotomy is not recommended, being sporadically indicated in cases of fetal distress, insufficient progress of labor and imminent injury of the 3rd and 4th degrees of the perineum. It is important for the nursing team to seek training regarding the practice in order to minimize the recurrence of episiotomy.Objetivo: Analisar a produção da enfermagem sobre o (des)uso da episiotomia no contexto da assistência ao parto. Metodologia: Revisão integrativa da literatura, realizada em outubro de 2022, a partir da análise de artigos disponíveis na íntegra entre os anos de 2017 a 2021, por meio da Biblioteca Virtual em Saúde (BVS), nas bases de dados Medical Literature Analysis and Retrieval System Online (MEDLINE), Literatura Latino-americana e do Caribe em Ciências da Saúde (LILACS), Base de dados em Enfermagem (BDENF), bem como na biblioteca eletrônica Scientific Electronic Library Online (SciELO). Resultados: Dentre as indicações para a realização do procedimento, se destacam: distocia, sofrimento fetal, macrossomia, período expulsivo prolongado e parto operatório. Constata-se a reafirmação quanto a não recomendação do procedimento por não apresentar evidências científicas suficientes e eficazes que defendam seu uso. Com relação as consequências, as mais citadas são: hemorragia, dispareunia, infecção, incontinência urinária e fecal, dor e disfunção sexual. Notou-se a defesa da humanização do parto através um atendimento transparente e eficaz que advém por meio da formação acadêmica dos profissionais, especialmente a enfermagem, no qual é responsável pelo acolhimento, apoio e orientação da mulher em todo processo de parto. Conclusão: a prática da episiotomia não é recomendada, sendo esporadicamente indicada nos casos de sofrimento fetal, progresso insuficiente do parto e lesão iminente de 3o e 4° graus do períneo. É importante que a equipe de enfermagem busque capacitação a respeito da prática com o objetivo de minimizar a recorrência de episiotomia
