Trend in admission and mortality of patients with stroke during COVID-19

Fear of COVID-19 infection and social distancing policies has impacted various aspects of daily living, and it has been reported that medical use of patients is decreasing. However, for diseases such as stroke, early recognition of the symptom and timely treatment has a major effect on the treatment prognosis. This study attempted to confirm whether the medical use and outcome of acute stoke patients changed by examining the number of stroke patients and mortality rate within 30 days of hospitalization during COVID-19 outbreak. Using a customized DB of the National Health Insurance Service, this study analyzed patients who hospitalized with stroke for main and minor diagnosis. Analyses in this report included cerebral infarction (I63), cerebral hemorrhage (I60-62), and TIA(Transient Ischemic attack, G45) based on ICD-10(International Classification of Diseases) code. The study period is a total of 39 weeks from January to August 2020. We compared it with the same week of 2017 to 2019, the period before the COVID-19 epidemic. In addition, it was divided into four Epoch(Epoch1 to Epoch4) in consideration of the spread trend of COVID-19 and analyzed trend by Epoch. The number of stroke hospitalized patients in 2020 was estimated by constructing a negative binomial regression model with data from 2017 to 2019, and compared with the actual number of patients. In the entire period(1-39 weeks), the number of stroke patients was 2.2% less than the predicted number of patients. During Epoch2 ('20.2.3.~5.2) and Epoch4 ('20.8.23.~10.3), the actual number of patients decreased significantly by 10.14% and 5.65% compared to the predicted number of patients. As a result of analysis by type of stroke, cerebral infarction patients exceeded the predicted number of patients by 0.69%, and the number of cerebral hemorrhage patients decreased by 1.28%, and the range of changes was not significant. However, in the Epoch2, the number of patients decreased by 7.71% and 5.74% respectively. In the entire period, the number of patients with TIA decreased by 15.10% compared to the predicted number of patients. As a result of analyzing the number of deaths within 30 days of stroke hospitalization in the same analysis method, 12.14% excess death of cerebral infarction patients occurred. And deaths of patients with cerebral hemorrhage was 2.77% less than the predicted number of deaths. After analyzing the logistic regression model with demographic factors, disease factors, medical institution factors, and medical use factors, the impact of the COVID-19 was confirmed. As a result of analyzing the deaths of cerebral infarction patients, there was no statistically significant difference in the risk of death in 2020 compared to 2019, but in the model excluding factors related patient’s severity, OR(the odds ratio) of death increased significantly to 1.139 in 2020. Considering the results of SSI(Stroke Severity Index) deteriorated in 2020, it is estimated that the severity that increased due to the COVID-19 epidemic affected the increase in mortality. Compared to 2019, OR of deaths in 2020 in patients with cerebral hemorrhage was 0.940, which was analyzed to have decreased the risk of death. During the COVID-19 epidemic, a decrease in the number of stroke patients was clearly observed and severity and mortality rates increased in cerebral infarction patients, especially when the spread was rapid. In the repeated epidemics occurrence such as MERS, SARS and COVID-19, unmet medical care and delayed treatment can lead to increased mortality as well as neurological disorders in patients with stroke. Changes in medical use patterns and outcomes should be continuously monitored and managed during the spread of infectious diseases.2020년 1월, 국내에서 COVID-19 첫 확진 환자가 발생한 이후 전국적으로 빠르게 확산되었다. 유행 초기, 감염에 대한 우려와 전파확산을 방지하기 위한 사회적 거리두기 정책의 영향으로 환자들의 의료 이용이 감소하였다는 연구들이 꾸준히 보고되고 있다. 그러나 뇌졸중과 같이 발병 이후 빠른 시간 안에 진단과 치료를 시작해야하는 응급질환들은 적시에 의료 이용을 하지 않거나, 이용 시점이 지연되면 환자 예후의 악화 등 치명적인 결과로 이어질 수 있다. 이에 본 연구에서는 COVID-19 유행 시기의 뇌졸중 환자 수, 입원 30일 이내 사망률을 분석하여 의료 이용 행태와 치료 결과를 살펴보고자 하였다. 국민건강보험공단 맞춤형 DB를 활용하였으며, 뇌졸중을 주・부진단으로 입원치료를 받은 환자를 연구 대상으로 설정하고, 뇌졸중은 ICD-10(국제질병분류) 코드를 기준으로 I63(뇌경색), I60~I62(뇌출혈), G45(일과성 뇌허혈발작)를 포함하였다. 2020년 1월부터 8월까지 39주 간의 데이터를 COVID-19 유행 이전 시기인 2017 ~2019년의 동일 시점과 비교하였다. 또한 COVID-19 확산 추세에 따른 변화를 반영하기 위해 39주를 4개 구간(Epoch1 ~Epoch4)으로 구분하고 구간별 분석을 시행하였다. COVID-19 유행 이전 3년(2017~2019년)의 주별 환자 수로 음이항 회귀모형(Negative Binomial Regression)을 시행하여 2020년 주별 예측 환자 수를 추정하고, 이를 2020년 실제 환자 수와 비교하였다. 1~39주까지 전체 기간에서 예측보다 2.2% 적은 환자들이 입원 치료를 받았다. 특히, COVID-19 대규모 집단 감염이 발생한 시기인 Epoch2 ('20.2.23.~5.2.)와 Epoch4('20.8.23.~10.3.) 기간에는 예측 환자 수와 각 10.14%, 5.65%의 격차를 보이며 더 큰 폭으로 감소하였다. 뇌졸중 종류별 분석 결과, 전체 구간에서 뇌경색 환자는 예측 환자 수를 0.69% 초과하였고, 뇌출혈 환자 수는 1.28% 적은 것으로 나타났다. 전체 뇌졸중 분석 결과와 유사하게 Epoch2 구간에서 각 7.71%, 5.74% 수준의 큰 폭으로 감소하였다. 뇌경색은 구간별로 지속적인 증가 혹은 감소 추세가 유지되었으나, 뇌출혈은 2~5주간 환자가 감소하였다가 비교적 이른 시점에 환자 수가 회복되는 특징적인 패턴이 관찰되었다. 중증도가 가벼운 TIA(일과성 뇌허혈발작) 입원 환자는 전체 기간 동안 예측 환자보다 15.10% 더 적었다. 뇌졸중 입원 30일 이내 사망자 수를 분석한 결과, 전체 구간에서 뇌경색 환자에서 12.14%의 초과 사망이 발생하였고, 뇌출혈은 예측한 것보다 2.77% 적었다. 로지스틱 회귀모형을 통해 COVID-19 유행이 사망에 미치는 영향을 분석하였을 때, 뇌경색 환자의 사망 분석에서는 2019년 대비 2020년 환자 사망 위험이 통계적으로 유의한 차이가 없었다. 그러나 중증도 관련 요인들을 제외한 모델에서는 사망 위험이 유의하게 증가하는 것으로 분석되었다. 2020년에 환자들의 중증도(SSI)가 악화되었다는 본 연구의 결과를 고려할 때, COVID-19 시기에 환자의 중증도가 높아짐에 따라 사망 위험도 증가한 것으로 해석할 수 있다. 뇌출혈 환자의 2019년 대비 2020년 사망 오즈비(OR)는 0.940으로 사망 위험이 오히려 감소한 것으로 분석되었다. COVID-19 유행 시기 중 특히 확산세가 컸던 시기에 뇌졸중 환자 수의 감소가 뚜렷하게 관찰되었고, 뇌경색 환자에서는 중증도와 특정 구간에서 사망률이 증가하였다.
