International audienceObjectiveTo provide guidelines for the management of the intensive care patient with severe acute pancreatitis.DesignA consensus committee of 22 experts was convened. A formal conflict-of-interest (COI) policy was developed at the beginning of the process and enforced throughout. The entire guidelines process was conducted independently of any industrial funding (i.e. pharmaceutical, medical devices). The authors were required to follow the rules of the Grading of Recommendations Assessment, Development and Evaluation (GRADE®) system to guide assessment of quality of evidence. The potential drawbacks of making strong recommendations in the presence of low-quality evidence were emphasized.MethodsThe last SFAR and CNGOF guidelines on the management of the patient with severe pancreatitis were published in 2001. The literature seems now sufficient for an update. The committee studied 14 questions within 3 fields. Each question was formulated in a PICO (Patients Intervention Comparison Outcome) format and the evidence profiles were produced. The literature review and recommendations were made according to the GRADE® methodology.ResultsThe experts’ synthesis work and the application of the GRADE® method resulted in 24 recommendations. Among the formalized recommendations, 8 have high levels of evidence (GRADE 1±) and 12 have low levels of evidence (GRADE 2±). For 4 recommendations, the GRADE method could not be applied, resulting in expert opinions. Four questions did not find any response in the literature. After one round of scoring, strong agreement was reached for all the recommendations.ConclusionsThere was strong agreement among experts for 24 recommendations to improve practices for the management of intensive care patients with severe acute pancreatitis.ObjectifÉmettre des recommandations pour la prise en charge du patient de soins critiques avec une pancréatite aiguë grave.ConceptionUn comité de 22 experts a été constitué. Une politique de déclaration et de suivi des liens d’intérêts a été appliquée et respectée durant tout le processus de réalisation du référentiel. De même, celui-ci n’a bénéficié d’aucun financement provenant d’une entreprise commercialisant un produit de santé (médicament ou dispositif médical). Le comité devait respecter et suivre la méthode GRADE® (Grading of Recommendations Assessment, Development and Evaluation) pour évaluer la qualité des données factuelles sur lesquelles étaient fondées les recommandations.MéthodesLes dernières recommandations SFAR sur la prise en charge du patient de réanimation avec une pancréatite aiguë ont été publiées en 2001. Nous avons souhaité réactualiser ces recommandations après analyse de la littérature selon la méthodologie GRADE® en identifiant les 3 champs suivants : évaluation et admission en soins critiques ; prise en charge initiale ; prise en charge des complications évolutives. Chaque question a été formulée selon le format PICO (Patients, Intervention, Comparison, Outcome).RésultatsLe travail de synthèse des experts et l’application de la méthode GRADE ont abouti à 24 recommandations. Parmi les recommandations, 8 ont un niveau de preuve élevé (GRADE 1±) et 12 un niveau de preuve faible (GRADE 2±). Pour 4 questions, la méthode GRADE ne pouvant pas s’appliquer, la recommandation s’appuie sur des avis d’experts. Il n’a pas été possible de statuer pour 4 autres questions.ConclusionVingt-quatre recommandations, avec un accord fort entre experts, sur la prise en charge des patients de soins critiques avec une pancréatite aiguë grave, ont été formulées
