Periodontal disease and adverse pregnancy outcomes : an umbrella review

Dissertação para obtenção do grau de Mestre no Instituto Universitário Egas MonizObjective: To appraise the methodological quality and the strength of evidence on the association between maternal periodontitis and adverse pregnancy outcomes (APOs). Materials & Methods: PubMed, Cochrane Database of Systematic Reviews, Web of Science, and LILACS were searched from inception to July 2022, without date or language restrictions. All systematic reviews reporting periodontal disease and APOs were included. The risk of bias was assessed through A MeaSurement Tool to Assess Systematic Reviews, and we estimated the meta-analytical strengths and validity of the evidence. The fail-safe number (FSN) was calculated for statistically significant meta-analyses. Results: The association of periodontal disease with APOs was explored on 26 systematic reviews, of which 14 conducted meta-analytical analysis. From the 28 estimates, periodontal disease had strong association with 3 APOs: preterm birth (PTB), low birth weight (LBW) and gestational diabetes mellitus (GDM). PTB and LBW revealed all the remaining strength levels, yet preeclampsia only showed suggestive and weak evidence. When analyzing the consistency of the significant estimates (weak to strong), only 8.7% estimates showed to be likely to change in the future, according to FSN statistics. None of the strong meta-analytical estimates had the possibility to change in the future, showing consistency. The impact of periodontal treatment effect on APOs were explored on 19 systematic reviews, of which 11 conducted meta-analysis. Overall, 42 meta-analyses were included and showed that periodontal treatment had no strong association with APOs. PTB revealed all the remaining strength levels, yet LBW only showed suggestive and weak evidence. Conclusion: Strong and highly suggestive evidence from observational studies support an association between maternal periodontitis and a higher risk of PTB, LBW, GDM and preeclampsia. The level of evidence on the effect of periodontal treatment to prevent APOs is still uncertain and requires future trials to draw definite and robust conclusions.Objetivo: Avaliar a qualidade metodológica e a força da evidência da associação entre periodontite materna e desfechos obstétricos negativos (DONs). Materiais e Métodos: A pesquisa foi realizada na PubMed, Cochrane Database of Systematic Reviews, Web of Science e LILACS até julho de 2022, sem restrições de data ou idioma. Foram incluídas as revisões sistemáticas que relacionassem doença periodontal e DONs. A avaliação do risco de viés foi realizada através da ferramenta A MeaSurement Tool to Assess Systematic Reviews, e determinámos a força meta-analítica e validade das evidências. O Fail-Safe Number (FSN) foi calculado para meta-análises estatisticamente significativas. Resultos: A associação entre doença periodontal e DONs foi explorada em 26 revisões sistemáticas, das quais 14 realizaram análise meta-analítica. Das 28 estimativas, a doença periodontal teve uma forte associação com 3 DONs: parto pré-termo (PPT), baixo peso ao nascer (BPN) e diabetes mellitus gestacional (DMG). PPT e BPN revelaram todos os níveis de força, mas a pré-eclâmpsia apenas mostrou evidências sugestivas e fracas. Ao analisar a consistência das estimativas significativas, concluiu-se que apenas 8,7% apresentavam probabilidade de mudar no futuro, de acordo com estatísticas FSN. Nenhuma das estimativas meta-analíticas fortes indicou a possibilidade de alteração no futuro, revelando consistência. O impacto do efeito do tratamento periodontal nos DONs foi explorado em 19 revisões sistemáticas, das quais 11 realizaram meta-análises. As 42 meta-análises incluídas mostraram que o tratamento periodontal não teve uma forte associação com DONs. PPT revelou todos os níveis de força, mas BPN apenas mostrou evidências sugestivas e fracas. Conclusão: Evidências fortes e sugestivas de estudos observacionais apoiam uma associação entre periodontite materna e um maior risco de PPT, BPN, DMG e pré-eclâmpsia. O nível de evidência sobre o efeito do tratamento periodontal na prevenção de DONs ainda é incerto e requer estudos futuros para tirar conclusões definitivas e robustas
