The impact of mechanical stimulus on the temporomandibular joint's cartilage : a systematic review

Dissertação para obtenção do grau de Mestre no Instituto Universitário Egas MonizIntroduction: The temporomandibular joint (TMJ)'s fibrocartilage develops in response to the local mechanical environment as a load-absorbing structure, essential for maintaining chondrocyte proliferation and extracellular matrix production. Several animal studies analyse this tissue’s response to different types of mechanical stimuli. Objectives: This systematic review intends to evaluate in vivo the morphological and cellular changes in TMJ’s fibrocartilage in response to different modalities and intensities of mechanical loading stimulus. Materials and Methods: PubMed, SCOPUS, and B-On databases were searched until December 2021. Hand-searching through citations was also performed. Only experimental in vivo studies that provided knowledge about the effect of the mechanical stimulus in TMJ’s fibrocartilage, published in the last ten years, were included. These were grouped into four categories: compressive force loading, anterior mandibular displacement, increased occlusal vertical dimension, and diet hardness. Random effects meta-analysis was also performed for the effect of compressive force loading on the cartilage thickness. Results: 25 articles were considered for the qualitative synthesis and 6 articles were included in the quantitative synthesis. Different dietary consistencies promoted differences in the thickness and histological content of fibrocartilage. Mandibular advancement was also associated with metabolic changes. The increase in vertical dimension revealed contradictory results. The compressive force loading led to a reduction of 0.49μm/g.day in the fibrocartilage thickness of rats, showing the initial age factor a significant effect on this wear (p<0.05). Conclusions: Mechanical stimulus significantly affects the temporomandibular joint’s fibrocartilage, showing not only morphological alterations but also changes in its cellular and metabolic activity, which seem to be related to the period and intensity of the stimulus. Clinical Relevance: Understanding how the temporomandibular joint’s fibrocartilage reacts to different modalities and intensities of mechanical stimulus plays an important role in orthodontic treatments that modify jaw growth and in the prevention of TMJ disease.Introdução: A fibrocartilagem da articulação temporomandibular (ATM) desenvolve-se em resposta ao ambiente mecânico local, atuando como uma estrutura de absorção de carga, essencial para a proliferação dos condrócitos, bem como para a manutenção e produção da matriz extracelular. São vários os estudos realizados em animais que analisam a resposta deste tecido a diversos tipos de estímulos mecânicos. Objetivos: Avaliar as alterações morfológicas e celulares que ocorrem na fibrocartilagem da ATM perante diferentes modalidades e intensidades de estímulo mecânico, in vivo. Materiais e Métodos: As bases de dados PubMed, SCOPUS e B-On foram pesquisadas até dezembro de 2021. Realizou-se também uma pesquisa manual. Apenas estudos in vivo nos quais foi avaliado o efeito do estímulo mecânico na cartilagem da ATM, escritos nos últimos dez anos foram incluídos. Agruparam-se estes em quatro categorias: carga compressiva, desvio anterior da mandíbula, aumento da dimensão vertical oclusal e consistência dietética. Foi também realizada uma meta-análise de efeitos aleatórios para o efeito da carga compressiva na espessura da cartilagem. Resultados: Foram incluídos 25 artigos na análise qualitativa e 6 na análise quantitativa. Diferentes consistências dietéticas promoveram diferenças na espessura e conteúdo histológico da fibrocartilagem. O avanço mandibular também se associou a alterações metabólicas. Já o aumento de dimensão vertical revelou resultados contraditórios.
