Evaluation of two mechanized file systems in the instrumentation of curved canals

The objective of this work was to compare the performance of the continuous rotation mechanized files ProTaper Next and TruNatomy (Dentsply Sirona Maillefer, Ballaigues, Switzerland) through canal wear, transportation and centering ability in curved canals, using CBCT.Materials and methods:Forty-three human mandibular molars were selected, from which 67 roots were prepared. These were divided according to the degree of curvature (straight, moderate and severe) and randomly distributed into 2 experimental groups, according to the instrumentation system used: ProTaper Next (PTN) and TruNatomy (TN). In the PTN group, the canals were instrumented with a K10 file and with the ProGlider, X1 and X2 files of the system. In the TN group, the canals were instrumented with the K10 file and with Orifice Modifier, Glider and Prime files of the system. Irrigation with 2.5% NaOCl was performed in both groups, between each instrument. Cone-Beam Computed Tomography images were obtained before and after instrumentation of the samples to analyze canal wear, transportation and centering ability in the apical (at 3mm from the apex) and middle (at 3mm from the apex) third. Values were measured using the Image J program and the results were statistically analyzed with p0.05). In severe curvatures, a PTN showed greater canal wear (p=0.038), a tendency towards greater transportation in the external direction of the curvature, and a better centering ability compared to a TN (p>0.05).In the middle third, in moderate curvatures, PTN showed greater canal wear (p=0.017) and a lower centering ability compared to TN (p>0.05). In severe curvatures, in PTN there was a tendency for greater canal wear (p=0.056) and a lower centering ability compared to TN (p>0.05).Discussion / Conclusion:Regarding TN, PTN showed greater wear in canals with moderate and severe curvatures and a tendency towards greater transportation in curved canals. Regarding the centering ability, there was a tendency towards greater centralization of PTN in the apical third and of TN in the middle third, with no statistically significant differences between the two systems.Introdução: A terapêutica endodôntica tem como objetivo a remoção dos tecidos pulpares infetados e uma conformação canalar que permita uma obturação adequada, e consequente bom prognóstico. O objetivo deste trabalho foi comparar o desempenho das limas mecanizadas em rotação contínua ProTaper Next e TruNatomy (Dentsply Sirona Maillefer, Ballaigues, Suíça) através do desgaste, do transporte do canal e da capacidade de centralização, em canais curvos, com recurso ao CBCT.Materiais e Métodos: Foram selecionados 43 molares mandibulares humanos, nos quais foram preparadas 67 raízes. Estas foram divididas consoante o grau de curvatura (reta, moderada e severa) e distribuídas aleatoriamente por 2 grupos experimentais, de acordo com o sistema usado: ProTaper Next (PTN) e TruNatomy (TN). No grupo da PTN, os canais foram instrumentados com a lima K10 e com as limas ProGlider, X1 e X2 do sistema. No grupo da TN, os canais foram instrumentados com a lima K10 e com as limas Orifice Modifier, Glider e Prime do sistema. A irrigação com NaOCl a 2,5% foi realizada em ambos os grupos, entre cada instrumento. As imagens de Tomografias Computorizadas de Feixe Cónico foram obtidas antes e após instrumentação das amostras para analisar o desgaste, o transporte e a centralização do canal no terço apical (a 3mm do ápice) e médio (a 5mm do ápice). Os valores foram medidos com o programa Image J e os resultados analisados estatisticamente com p0,05). Nas curvaturas severas, a PTN mostrou maior desgaste do canal (p=0,038), uma tendência para maior transporte na direção externa da curvatura e melhor capacidade de centralização comparativamente à TN (p>0,05).No terço médio, nas curvaturas moderadas, a PTN mostrou maior desgaste do canal (p=0,017) e menor capacidade de centralização comparativamente à TN (p>0,05). Nas curvaturas severas, na PTN verificou-se uma tendência para maior desgaste do canal (p=0,056) e menor capacidade de centralização comparativamente à TN (p>0,05). Discussão/ Conclusão: Relativamente à TN, a PTN apresentou maior desgaste em canais com curvaturas moderadas e severas e uma tendência para um maior transporte em canais curvos. Relativamente à capacidade de centralização, houve uma tendência de maior centralização da PTN no terço apical e da TN no terço médio, sem diferenças estatisticamente significativas entre os dois sistemas
