The study aimed to show the impact Monitoring and Evaluation system on Malnutrition reduction to under 5 children in Wanging’ombe district because it is among the council in Njombe region where malnutrition is high about 42.3% (WDC, 2019). The study involved sample size 98 health workers from selected health facilities and CHMT members. Data of this study was collected by using interview and documentary review and tools used were questionnaire and checklist, sampling techniques used for the study was purposive and convenience sampling.
