Le McGrath™ au bloc opératoire : étude observationnelle rétrospective des facteurs associés à son utilisation

Introduction: The use of a video laryngoscope (VL) is an alternative to direct laryngoscopy (DL) for tracheal intubation. Among the VL available on the market, the McGrath® MAC (MG) seems to be the most used in operating rooms. The French Society of Anesthesia and Intensive Care (SFAR) recommends VL only for difficult intubations. However, it is common to see the MG used as a first line method, outside of this case.Objectives: The main objective of this study is to identify factors associated with the use of MG.Methods: An observational and retrospective study was conducted at the Cavale Blanche hospital (CHU de Brest). Anesthesia charts, concerning patients who underwent surgery under general anesthesia with tracheal intubation at the beginning of 2017, were analyzed. Three kinds of data were collected: patient’s predictive factors of difficult intubation, other variables that may influence device choice, and finally, the technique used (MG or DL).Results: 146 charts were analyzed. Patients with ≥ 2 predictive factors for difficult intubation (odds ratio [OR] = 5.7, 95% confidence interval [CI]= 1.9,17.4), emergency management (OR= 4.2, 95% CI= 1.9,9.6), and presence of fragile teeth (OR= 5.8, 95% CI= 2.6,12.9), were variables associated with the use of the MG.Conclusions: The findings of this research indicate that in addition to predictive factors of difficult intubation, fragile teeth, emergency management, and difficult or not performed mask ventilation, are other factors associated with the use of MG.Introduction : Le McGrath™ est un vidéolaryngoscope (VL) utilisé comme alternative à la laryngoscopie directe pour l’intubation orotrachéale. La SFAR (Société Française d’Anesthésie Réanimation) réserve le recours aux VL aux cas d’intubation difficile. Pourtant, il est courant de voir le MG utilisé en première intention, en dehors de ce cas de figure. Objectifs : Identifier les facteurs associés au choix du MG pour une intubation. Matériels et méthodes : Une étude observationnelle rétrospective a été menée à l’hôpital de la Cavale Blanche du CHU de Brest. Les dossiers d’anesthésie de patients ayant été opérés sous anesthésie générale avec intubation orotrachéale au début de l’année 2017 ont été analysés. Les facteurs prédictifs d’intubation difficile, les autres facteurs pouvant influencer le choix de la technique, et enfin la technique utilisée étaient relevés. Résultats : 146 dossiers ont été inclus. Les variables associées de façon significative à l’utilisation du MG sont : facteurs d’intubation difficile ≥2 (Odds Ratio (OR) = 5,7. Intervalle de confiance à 95% (IC95) = [1,9 ;17,4]), une prise en charge en urgence (OR= 4,2. IC95 = [1,9 ;9,6]), la présence de dent(s) fragile(s) (OR = 5,8. IC95 = [2,6 ;12,9]), et une ventilation au masque difficile ou non réalisée (OR= 2,5. IC95= [1,2 ;5,4]). Conclusion : En plus des facteurs prédictifs d’intubation difficile, la présence de dents fragiles, une prise en charge en urgence, ou une ventilation au masque difficile ou non réalisée, sont d’autres facteurs associés à l’utilisation du MG
