Distal radius fractures: Agreement study in the evaluation of images and treatment selection comparing of x-ray vs CT scan

Introducción: Las fracturas de radio distal son la fractura más común en huesos largos. Su evaluación se realiza con radiografías, un recurso económico y ampliamente disponible. La tomografía axial computarizada (TAC) permite aumentar el detalle anatómico, en especial del compromiso intraarticular y la conminución. Sin embargo, representa un costo adicional no despreciable, calculado entre el 15 al 30%. Existe controversia si es necesaria para mejorar la interpretación de estas fracturas y la selección de tratamiento. Métodos: Se realizó un estudio de concordancia de imágenes diagnósticas (radiografías y TAC) para la evaluación de fracturas del radio distal y selección del tratamiento. Se analizó la valoración hecha a 72 casos por cirujanos de mano experimentados (más de diez años de experiencia) como patrón de oro, en relación con las de cuatro evaluadores de diferente experticia (dos ortopedistas generales y dos cirujanos de mano). Cada uno realizó individualmente evaluación en dos fases, una inicial con imágenes de radiografías y una final con disponibilidad de las imágenes de radiografías y tomografía. Resultados y conclusiones: Las imágenes de TAC para la evaluación de las fracturas de radio distal son un elemento fundamental en la medición más acertada de longitudes (brechas y escalones). La radiografía simple tiene un mejor desempeño para la medida de ángulos (inclinación palmar e inclinación radial). Sin embargo, aun cuando la evaluación de TAC permite una clasificación de la fractura y selección de tratamiento mejoradas hasta en un 17,5% y 7,5% respectivamente; es evidente como la selección del tratamiento tiene gran variabilidad inclusive en grupos de mayor experticia.Introduction: Distal radius fractures are the most common fracture in long bones. Its evaluation is done with x rays, an economic and widely available resource. Computed axial tomography (CT scan) allows greater anatomical detail, especially of intra-articular compromise and comminution. However, it represents a non-negligible additional cost, calculated between 15 to 30%. There is controversy as to whether it is necessary to improve the assessment of these fractures and the selection of treatment. Materials and methods: A study of concordance of diagnostic images (x-rays and CT scan) was carried out for the evaluation and selection of treatment of distal radius fractures. We analyze the assessment of 72 cases done by experienced hand surgeons (more than ten years of experience) as a gold standard, in relation to those of four additional participants with different expertise (two general orthopedic surgeons and two hand surgeons). Each individual carried out an evaluation in two phases, an initial one with X-ray images and a second stage with the availability of X-ray and tomography images. Results and conclusions: CT scan images for the evaluation of distal radius fractures are a fundamental element for a more accurate measurement of lengths (gaps and steps). Simple radiography has a better performance for the measurement of angles (palmar inclination and radial inclination).
