Ethical analysis of the limitations to the ability to tell and narrate the experience of illness and suffering in doctor-patient communication

Con la realización de este estudio ha sido posible analizar algunos aspectos de la capacidad de narrar del ser humano y del lenguaje en uso de la comunicación en el encuentro medico-paciente, entendido éste como una actividad moral que acontece en una relación humana donde los pacientes, además de la resolución de los problemas de salud, esperan del médico una escucha atenta de sus narrativas y relatos de sus historias de enfermedad considerados, a veces, desde la perspectiva biomédica, como innecesarios y erróneos. Asimismo, se ha podido poner de relieve la pertinencia de reflexionar sobre algunos aspectos de la pragmática de la comunicación en la relación del médico con el enfermo -en el contexto del progreso y el desarrollo biotecnológico y los éxitos terapéuticos alcanzados- desde la mirada del paciente y su historia de aflicción, enfermedad y sufrimiento y no solo desde el discurso hegemónico de la biomedicina donde el paciente en la más de las veces no es tenido en cuenta. De otra parte, y desde la perspectiva fenomenológico-hermenéutica de Paul Ricoeur, se tuvo la oportunidad de reflexionar desde la lingüística la dimensión significante y sociocultural del enfermo, su biografía, su mundo local y sus condiciones materiales de existencia. Igualmente, se llega a la comprensión del paciente como texto con capacidad de actuar, pensar, hablar, narrar, sufrir, de que se le imputen acciones y responsabilidades, con aspiración a la verdadera vida con y para el otro en instituciones justas (Ricoeur 1990). También se ha pretendido contrastar en las entrevistas realizadas el valor terapéutico del relato biográfico en la historia clínica, pensando en que el paciente no solo es un narrador sino también lector de su propia historia, siempre y cuando no sea hecha en un contexto exclusivamente clínico ausente de discursos y expresiones de aflicción y omitiendo la voz del enfermo.This study analyzes some aspects of the narrative capacity of the human being and language in use of communication in the doctor-patient encounter. This encounter is understood as a moral activity that takes place in a human relationship where patients, in addition to the resolution of health problems, expect the physician to listen thoughtfully to their narratives and accounts of their stories of illness, sometimes considered, from the biomedical perspective, as unnecessary and erroneous. Furthermore, in the context of biotechnological development advancements and therapeutic successes, the study highlights the importance of reflecting on the pragmatics of communication between the physician and the patient from the former's point of view. In particular, considering the patients' history of affliction, illness, and suffering and not only from the hegemonic discourse of biomedicine where the patient is more often than not taken into account. On the other hand, and from the phenomenological-hermeneutic perspective of Paul Ricoeur, we had the opportunity to reflect from linguistics on the significant and sociocultural dimension of the patient, his biography, his local world, and his material conditions of existence. Likewise, the understanding of the patient as a text with the capacity to act, think, speak, narrate, suffer, to have actions and responsibilities imputed to him, with the aspiration to an authentic life with and for the other in just institutions (Ricoeur 1990). It was also intended to contrast, through the conducted interviews, the therapeutic value of the biographical account in the clinical history, considering that the patient is not only a narrator but also a reader of his own story. However, to assume that role as a narrator, his story can not be discoursed in an exclusively clinical context, absent of speeches and expressions of distress and omitting the patient's voice.Magíster en BioéticaMaestrí
