Malocluzia — aspecte genetice

Background. Dental malocclusion or dento-maxillary anomalies means an abnormal teeth relation between upper and lower jaw, influenced by genetics and environmental factors. Objective of the study. The study of genetic aspects in the development of malocclusions. Material and Methods. Data from the scientific literature have been studied and analyzed using the PubMed, Google Scholar, Hinari databases. Results. The heritability coefficient higher than 60% and family aggregation confirm the genetic nature of malocclusions, the multifactorial determinism of dento-maxillary anomalies, but also the autosomal-dominant heredity of some types (malocclusion class II and III). Approximately 150 genes / loci are associated with craniofacial abnormalities with malocclusions. Candidate genes involved in bone development have been described: FGFR2, COL1A1, PAX9, TGFB3, LTBP, IGF1, ENPP1, EVC, RUNX2 and NOG; genes involved in cartilage development: GHR and Matrilin1; genes involved in muscle development: MYO1H, ACTN3, CAT6B and DUSP6; genes responsible for tooth morphogenesis: EDA, XEDAR and BMP2. Some genes expressed in facial soft tissue: ARHGAP29, PRDM16, PAX3, TP63, C5orf50, Col17A1, HMGA2, AJUBA and ADK are responsible for facial symmetry, face width and height. Conclusion. The correct diagnosis of the etiology of malocclusion with hereditary determinism represents the success of orthodontic treatment, individualizing the treatment plan specific to the clinical case and the most accurate prognosis of long-term maxillofacial changes.Introducere. Malocluziile dentare sau anomaliile dento- maxilare reprezintă o relație anormală între dinții celor două arcade dentare, determinate de factorii genetici în interacțiune cu cei de mediu. Scopul lucrării. Studiul unor aspecte genetice în dezvoltarea malocluziilor. Material și metode. Au fost studiate și analizate datele din literatura științifică, utilizând bazele de date PubMed, Google Scholar, Hinari. Rezultate. Coeficientul de heritabilitate mai mare de 60% și agregarea familială confirmă natura genetică a malocluziilor, determinismul multifactorial al anomaliilor dento-maxilare, dar și ereditatea autosomal-dominantă a unor tipuri (malocluzie clasa II și III). Aproximativ 150 de gene/ loci sunt asociate cu anomalii cranio-faciale ce prezintă malocluzii. Au fost descrise gene canditate, implicate în dezvoltarea oaselor: FGFR2 , COL1A1, PAX9, TGFB3, LTBP, IGF1, ENPP1, EVC, RUNX2 și NOG; gene implicate în dezvoltarea cartilajului: GHR și Matrilin1; gene implicate în dezvoltarea musculară: MYO1H, ACTN3, CAT6B și DUSP6; gene responsabile de morfogeneza dinților: EDA, XEDAR și BMP2. Unele gene exprimate în țesutul moale facial: ARHGAP29, PRDM16, PAX3, TP63, C5orf50, Col17A1, HMGA2, AJUBA și ADK sunt responsabile de simetria facială, de lățimea și înălțimea feței. Concluzii. Diagnosticul corect al etiologiei malocluziei cu determinism ereditar reprezintă succesul tratamentului ortodontic, individualizând planul de tratament specific cazului clinic și prognosticul cât mai corect al modificărilor de creștere maxilo-facială pe termen lung
