La médecine générale en trans-ition : étude qualitative par entretiens semi-dirigés de médecins généralistes de Haute-Savoie sur leur implication dans le parcours de soins des patients transgenres

Introduction : considering the increase in the number of transgender patients, political and medical institutions wish to involve general practitioners more in order to promote access to care. This study asked general practitioners about the role they attribute to themselves in the care of transgender patients. Method : qualitative study by semi-directed interviews conducted with eleven general practitioners based in Haute-Savoie (74). Analysis of the results by codification of verbatim statements using the N Vivo 1.6.1 software and triangulation. Findings : the general practitioners in Haute-Savoie interviewed were open to the question of transidentity but identified limits in the role they assigned to themselves in care. On the medical level, they were involved in the overall care of the patient. They described many difficulties related to the lack of training such as mastering the specific vocabulary of trans identity, the management of hormone therapy and knowledge of the network. Sexuality, although a source of confusion with gender identity, was not systematically mentioned. Very few discussed parenthood. On the psychological level, the participants invested themselves in the accompaniment without explicitly mentioning discrimination and vulnerabilities (anxio-depressive disorder, eating disorder, addictive disorders). The low number of doctors mentioning psychiatric care testified to the process of “depsychiatrisation” invoked by patients and militant associations such as RITA, OUTrans, Chrysalide. On the social level, no one mentioned the existence of the ALD. They were interested in the transgender patient’s circle of friends and family and their socio-professional integration. Only a few participants mentioned patient associations as a potential resource in the face of all these difficulties. Conclusion : the general practitioners interviewed had a rather positive feeling with regard to transidentity. They were involved in the care of transgender patients but described difficulties. They did not grasp the impact of these difficulties on the doctor-patient relationship. This could explain the lack of interest in further training.Introduction : compte-tenu de l’augmentation du nombre de patients transgenres, les institutions politiques et médicales souhaitent impliquer davantage les médecins généralistes afin de favoriser l’accès aux soins. Cette étude les a interrogés sur le rôle qu’ils s’attribuent dans la prise en charge des patients transgenres. Méthode : étude qualitative par entretiens semi-dirigés conduits auprès de onze médecins généralistes installés en Haute-Savoie (74). Analyse des résultats par codification de verbatims à l’aide du logiciel N Vivo 1.6.1 et triangulation. Résultats : les médecins généralistes de Haute-Savoie interrogés étaient ouverts à la question de la transidentité mais identifiaient des limites dans le rôle qu’ils s’attribuaient dans la prise en charge. Sur le plan médical, ils s’impliquaient dans la prise en charge globale du patient. Ils ont décrit de nombreuses difficultés liées à l’absence de formation telles que la maîtrise du vocabulaire spécifique de la transidentité, la gestion de l’hormonothérapie et la connaissance du réseau. La sexualité, pourtant source d’une confusion avec l’identité de genre, n’a pas été systématiquement évoquée. Très peu ont abordé la parentalité. Sur le plan psychologique, les participants s’investissaient dans l’accompagnement sans évoquer explicitement les discriminations et vulnérabilités (trouble anxiodépressif, trouble du comportement alimentaire, troubles addictifs). Le faible nombre de médecins évoquant une prise en charge psychiatrique témoignait du processus de « dépsychiatrisation » revendiqué par les patients et les associations militantes telles que RITA, OUTrans, Chrysalide. Sur le plan social, aucun n’a évoqué l’existence de l’ALD. Ils se sont intéressés à l’intégration socio-professionnelle du patient transgenre. Seulement quelques participants ont évoqué les associations de patients comme ressource potentielle face à l’ensemble de ces difficultés. Conclusion : les médecins généralistes interrogés avaient un ressenti plutôt positif à l’égard de la transidentité. Ils s’impliquaient dans la prise en charge des patients transgenres mais décrivaient des difficultés. Ils n’appréhendaient pas l’impact de ces difficultés sur la relation médecin-patient. Cela pourrait expliquer le manque d’intérêt pour des formations complémentaires
