The authors, after a short introduction on historical role of surgery and microsurgery in lymphatic vessel pathology, report their own 25-year experience in lymphatic microsurgery and lymphatic-venous-lymphatic anastomoses. In particular, the authors describe the derivative and reconstructive microsurgical techniques, pointing out the preoperative diagnostic, the indications and the obtained results concerning 798 patients affected by primary and secondary limb lymphedema
