Vaginosis bacteriana, una enfermedad que se rebela a los postulados de Koch y, el rol del microbioma vaginal en su desarrollo

Bacterial vaginosis are infections that affect 30% of the women in reproductive age. It’s a not well understood, common disease, that its mostly treated without clear cause. The vaginosis is a process caused by the disbiosis of vaginal microbiome, in which the disruption of the ecosystem causes inflammation and shift from lactobacillus to other microorganisms. Different risk factors, environmental determinants, and the individual, promote vaginal disbiosis. Even though different microorganisms have been associated to the disease, there is no etiologic agent. Metagenomic studies, had determined that, microorganisms no detected by normal methods, like bacterial culture, are associated to the disease. The detection of this agents in the general population, may change the knowledge about the vaginal microbiome, and determine the diagnosis of bacterial vaginosis. In conclusion, that there’s a small amount of information of vaginal microbiome, but a lot to investigate and comprehend. This must be an incentive for professionals, and their job in making questions and finding answers, in order to generate new efficient treatments.Las vaginosis bacterianas son infecciones que afectan a un 30% de las mujeres en edad reproductiva. Es una enfermedad común, pero poco entendida, que muchas veces es tratada sin tener claridad de su causa. Las vaginosis, son procesos causados por disbiosis en la microbiota vaginal, que rompen el equilibrio en el ecosistema, generando inflamación y desplazamiento de los lactobacilos. Diferentes factores de riesgo y determinantes ambientales o del individuo, promueven la disbiosis vaginal. Aunque se han asociado ciertos microorganismos, no se conoce un agente etiológico. Algunos estudios han tenido como objetivo, estudiar las diferentes variables que influyen en la disbiosis, y, análisis metagenómicos han determinado, que existen microorganismos no identificados, asociados a la enfermedad. Conocer cuáles son esos microorganismos, detectarlos en la población y cambiar nuestra mentalidad sobre la microbiota vaginal, podría afinar el diagnóstico de las vaginosis bacterianas. Se concluye que el conocimiento de la microbiota vaginal está en sus inicios, hay mucho que investigar y comprender. Esto debe ser un incentivo para que los profesionales se hagan preguntas, busquen respuestas y se generen nuevos tratamientos eficaces
