Symptom clusters of patients with colorectal cancer: a study of the determinants of their lived experience

Background: Cancer patients experience a large number of co-occurring symptoms. The concept of the Symptom Complex (SC) has clarified synergistic relationships between symptoms and underlying biological mechanisms. The relevant studies are quantitative. It is worth investigating whether a qualitative method can be used to identify a patient's SS, and the relationship of SS with markers of inflammation. Purpose: The objectives of the present study were: 1) Identification of multiple symptoms that occur simultaneously and organized in SCs, with a quantitative and qualitative method, 2) Investigation of the determinants that influence the experience of symptoms, 3) Investigation of the relationship between the severity of the symptoms of a SC with laboratory markers of inflammation. Method: The study is cross-sectional mixed methodology. Sampling was purposive, involving quantitative data of 64 outpatients under chemotherapy with a documented diagnosis of colorectal cancer. A blood sample was taken in the first and fourth cycles of chemotherapy before the administration of cytostatic drugs, to measure CRP, cytokines (IL-6, IL-8, IL17A, TGFβ), oxytocin and white blood cells (neutrophils-lymphocytes). Patients completed two questionnaires (System of Beliefs Inventory _SBR-15 and Life Orientation Test Revised _LOT-R), to assess religiosity and optimism respectively. The evaluation of symptoms was done in the fourth cycle of HCV, with the MD Anderson Symptom Inventory (MDASI) scale. Twenty-three (23) of the 64 patients agreed to give a semi-structured interview. The statistical package SPSS 22 was used for the identification of SCs and determinants of the experience, and as methods correlation test, principal component analysis and cluster analysis. The analysis of the qualitative data followed the interpretive phenomenological approach. Results: Prevalence and severity data of 14 symptoms were used to identify SCs by principal component analysis and cluster analysis. The same number of SCs was recognized with a slight variation of the composition. The SCs identified by principal component analysis (PCA) using the severity data were: 1) Vomit-nausea-xerostomia-anorexia, 2) Sadness-anxiety, 3) Peripheral neuropathy-sleep disturbance-memory problems and 4) Drowsiness-fatigue- pain. Cluster analysis for differences in symptom severity between patients distinguished three groups of patients with different symptom profiles. There were differences between patients with severe disease and patients with mild disease, in terms of cytokine concentration and individual and clinical characteristics. Principal component analysis and correlation testing confirmed that age was the most important determinant of SCs composition. The analysis of quantitative and qualitative data elucidated the importance of the SC sadness-anxiety for the experience of patients with colorectal cancer. There was a correlation between inflammatory markers and symptoms' severity. IL-17A was associated with fatigue-memory problem, TGFβ with dry mouth while oxytoxin had a positive association with symptom pair peripheral neuropathy-sleep disturbance and a negative association with SC sadness- anxiety. The cytokine IL-8 was not associated with symptoms' severity, but with a high tumor burden. Patients during interviews described associations between symptoms. They described nuclear symptoms common to those of quantitative analysis. SCs were identified for each patient, based on correlations between symptoms and time of onset. Interpretive phenomenological analysis revealed the following categories of themes: symbolic language, self-transcendence, cancer meaning-making, coping mechanisms, suffering, and symptoms' impact. Significance of the study - limitations: The present study is a first attempt to study the relationships between symptoms in the Greek cancer population, using the Symptom Cluster as a tool. The main limitation of the study was the organization of SCs with different criteria in the quantitative and the qualitative approach which makes the comparison of SCs difficult. Conclusion: Despite the identification of SCs with different criteria in the quantitative and qualitative part of the study, the most frequent and stable SC is the sadness-anxiety pair. The negative association between optimism and this symptom cluster advocates for the design of cognitive nursing interventions targeting the cluster, to reduce the distress of all symptoms. Related studies had positive results. The negative association of sadness-anxiety with oxytocin is consistent with recent data for the association of the hormone with neuropsychiatric disorders, as a result of its central action in the CNS. There is correlation of cytokines with symptom severity eg TGFβ-xerostomia, IL17A with fatigue-memory problems. The value of measuring inflammatory markers prior to initiation of chemotherapy to predict symptoms needs to be further investigated. The initial measurement of IL-8 is indicative of the extent of the disease and may be used as diagnostic biomarker.Υπόβαθρο: Οι ασθενείς με καρκίνο βιώνουν μεγάλο αριθμό συμπτωμάτων που εμφανίζονται ταυτόχρονα. Η έννοια του Συμπλέγματος Συμπτωμάτων (ΣΣ) έχει συντελέσει στην κατανόηση των συνεργικών σχέσεων μεταξύ των συμπτωμάτων και των υποκείμενων βιολογικών μηχανισμών. Οι σχετικές μελέτες είναι ποσοτικές. Αξίζει να διερευνηθεί αν μπορεί να χρησιμοποιηθεί ποιοτική μέθοδος για την αναγνώριση των ΣΣ ενός ασθενή, και η σχέση των ΣΣ με δείκτες φλεγμονής. Σκοπός: Οι στόχοι της παρούσας μελέτης ήταν: 1) Αναγνώριση πολλαπλών συμπτωμάτων που εκδηλώνονται ταυτόχρονα και οργανώνονται σε ΣΣ, με ποσοτική και ποιοτική μέθοδο, 2) Διερεύνηση των προσδιοριστών που επηρεάζουν το βίωμα των συμπτωμάτων, 3) Διερεύνηση της σχέσης της σοβαρότητας των συμπτωμάτων ενός ΣΣ με εργαστηριακούς δείκτες φλεγμονής. Μέθοδος: Η μελέτη είναι συγχρονική μικτής μεθοδολογίας. Η δειγματοληψία ήταν σκόπιμη, αφορούσε ποσοτικά δεδομένα 64 εξωτερικών ασθενών υπό ΧΜΘ με τεκμηριωμένη διάγνωση καρκίνου ΠΕ. Έγινε λήψη δείγματος αίματος στον πρώτο και τέταρτο κύκλο ΧΜΘ πριν την χορήγηση των κυτταροστατικών φαρμάκων για μέτρηση CRP, κυτοκινών (IL-6, IL-8,IL17A, TGFβ), οξυτοκίνης και λευκών αιμοσφαιρίων (ουδετερόφιλα- λεμφοκύτταρα). Οι ασθενείς συμπλήρωσαν δύο ερωτηματολόγια (System of Beliefs Inventory _SBR-15 και Life Orientation Test Revised _LOT-R), για την αξιολόγηση της θρησκευτικότητας και της αισιοδοξίας αντίστοιχα. Η αξιολόγηση των συμπτωμάτων έγινε στον τέταρτο κύκλο ΧΜΘ, με την κλίμακα MD Anderson Symptom Inventory (MDASI). Είκοσι τρεις (23) από τους 64 ασθενείς δέχτηκαν να δώσουν ημιδομημένη συνέντευξη. Για την αναγνώριση των ΣΣ και των προσδιοριστών του βιώματος χρησιμοποιήθηκε το στατιστικό πακέτο SPSS 22 και ως μέθοδοι έλεγχος συσχέτισης, ανάλυση κύριας συνιστώσας και ανάλυση κατά συστάδες. Η ανάλυση των ποιοτικών δεδομένων ακολούθησε την ερμηνευτική φαινομενολογική προσέγγιση. Αποτελέσματα: Τα δεδομένα επιπολασμού και σοβαρότητας 14 συμπτωμάτων χρησιμοποιήθηκαν για την αναγνώριση ΣΣ με ανάλυση κύριας συνιστώσας και ανάλυση κατά συστάδες. Προέκυψε ο ίδιος αριθμός ΣΣ με μικρή διαφοροποίηση της σύστασης. Τα ΣΣ που αναγνώρισε η ανάλυση κύριας συνιστώσας (PCA) χρησιμοποιώντας τα δεδομένα σοβαρότητας ήταν : 1) εμετός-ναυτία-ξηροστομία-ανορεξία, 2) Θλίψη- άγχος, 3) Περιφερική νευροπάθεια-διαταραχή ύπνου προβλήματα μνήμης και 4) Υπνηλία-κόπωση-πόνος. Στατιστικός έλεγχος με ανάλυση κατά συστάδες για διαφορές ως προς την σοβαρότητα των συμπτωμάτων μεταξύ των ασθενών, διέκρινε τρεις ομάδες ασθενών με διαφορετικό προφίλ συμπτωμάτων. Υπήρχαν διαφορές μεταξύ ασθενών με σοβαρό βίωμα και ασθενών με ήπιο βίωμα, ως προς τη συγκέντρωση των κυτοκινών και τα ατομικά και κλινικά χαρακτηριστικά. Η ανάλυση κύριας συνιστώσας και ο έλεγχος συσχέτισης επιβεβαίωσαν, ότι η ηλικία ήταν ο πιο σημαντικός προσδιοριστής της σύστασης των ΣΣ. Η αισιοδοξία επηρεάζει την αξιολόγηση της σοβαρότητας θλίψης-άγχους. Η ανάλυση ποσοτικών και ποιοτικών δεδομένων ανέδειξαν την κεντρική σημασία του ΣΣ θλίψη-άγχος για το βίωμα των ασθενών με καρκίνο ΠΕ. Υπήρχε συσχέτιση δεικτών φλεγμονής και σοβαρότητας συμπτωμάτων. Η IL-17A συσχετίστηκε με κόπωση-πρόβλημα μνήμης, ο TGFβ με ξηροστομία ενώ η οξυτοκίνη είχε θετική συσχέτιση με περιφερική νευροπάθεια - διαταραχή ύπνου και αρνητική με το ΣΣ άγχος-θλίψη . Η κυτοκίνη IL-8 δεν συσχετίστηκε με τη σοβαρότητα των συμπτωμάτων, αλλά με υψηλό καρκινικό φορτίο. Οι ασθενείς κατά τη διάρκεια των συνεντεύξεων περιέγραψαν συσχετίσεις μεταξύ των συμπτωμάτων και πυρηνικά συμπτώματα κοινά με αυτά της ποσοτικής ανάλυσης. Έγινε αναγνώριση ΣΣ για κάθε ασθενή, με κριτήριο τις συσχετίσεις μεταξύ συμπτωμάτων και το χρόνο εκδήλωσης. Η ερμηνευτική φαινομενολογική ανάλυση είχε ως αποτέλεσμα τις εξής κατηγορίες θεμάτων: συμβολική γλώσσα, αυτοϋπερβατικότητα, νοηματοδότηση του καρκίνου, μηχανισμοί αντιμετώπισης, υποφέρειν και επίδραση των συμπτωμάτων.Σημαντικότητα μελέτης- περιορισμοί : Η παρούσα μελέτη αποτελεί μια πρώτη προσπάθεια μελέτης στον ελληνικό πληθυσμό των σχέσεων μεταξύ των συμπτωμάτων ασθενών με καρκίνο ΠΕ, χρησιμοποιώντας ως εργαλείο το ΣΣ. Ο κύριος περιορισμός της μελέτης ήταν η οργάνωση των συμπτωμάτων των ΣΣ με διαφορετικά κριτήρια στην ποσοτική και στην ποιοτική προσέγγιση που κάνει δύσκολη τη σύγκριση των ΣΣ . Συμπέρασμα: Το πιο συχνό και σταθερό σύμπλεγμα των ασθενών ήταν το ΣΣ θλίψη-άγχος. Η αρνητική συσχέτιση αυτού του ΣΣ, με την αισιοδοξία και την οξυτοκίνη, συνηγορεί υπέρ του σχεδιασμού γνωστικών νοσηλευτικών παρεμβάσεων στοχευμένων σε αυτό. Η αρνητική συσχέτιση του ΣΣ με την οξυτοκίνη συνάδει με πρόσφατες μελέτες για τη συσχέτιση της ορμόνης με νευροψυχιατρικές διαταραχές, ως αποτέλεσμα της κεντρικής δράσης της στο ΚΝΣ. Υπάρχει συσχέτιση κυτοκινών με τη σοβαρότητα συμπτωμάτων π.χ.TGFβ-ξηροστομία, IL17A με κόπωση-πρόβλημα μνήμης. Πρέπει να διερευνηθεί περαιτέρω η αξία μέτρησης δεικτών φλεγμονής πριν την έναρξη ΧΜΘ για την πρόβλεψη των συμπτωμάτων. Η αρχική μέτρηση της IL-8 είναι ενδεικτική της έκτασης της νόσου και ίσως μπορεί να αξιοποιηθεί στη διάγνωση
