The effect of locally injected autologous platelet rich plasma on second intention wound healing of skin defects and survival of subdermal plexus skin flaps in cats: experimental study

Skin defects in cats usually occur due to trauma, oncologic procedures or skin infections. Healing of skin defects in cats can be accomplished by first or second intention or by using flaps or drafts. The objective of the present study was to evaluate the efficacy of platelet-rich plasma (PRP) infiltration in cutaneous deficits and subdermal plexus flaps in cats. Three full-thickness skin defects 2X2 cm were created bilaterally in the dorsal midline of eight laboratory cats, with the wounds of one side designated as the PRP group and the wounds of the other side as the control group. One flap, 2 cm wide and 6 cm long, was also created bilaterally in the dorsal midline. One flap was treated with PRP and the other served as control. Wound and flap healing was evaluated by clinical examination daily, as well as by photoplanimetry, Laser Doppler flowmetry and histological examination on days 0, 7, 14, and 25. Measurements of metalloproteinases (MMPs)-2 and -9 and tissue inhibitor metalloproteinase (TIMP)-1 on days 0, 14, and 25 were also performed. Based on the results of the present study, the mean time for full coverage of the wound with granulation tissue was shorter in the PRP group (10.88 ± 1.26 days) compared to the control group (12.13 ± 1.28) [p=0.008]. The mean % percentage of contraction was increased in the PRP compared to the control group (52.02% and 40.75%, respectively) [p=0.002], as well as the mean % percentage of total wound healing (68.51% and 61.19%, respectively) [p=0.002]. Finally, the mean tissue perfusion value was higher in the PRP group (2.55 mm/sec) compared to control (2.11 mm/sec) during the whole examination period (p<0.005). On day 14, the mean survival of PRP flaps was 80.437 % (±22.745) and of control flaps 66.516 % (±24.12), but no significant difference between the two groups was noted. Histologically, on day 25, the base of the PRP flap showed less edema score compared to the control flap (p=0.034). In conclusion, this is the first study focusing on the topical application of PRP in cutaneous deficits and flaps in cats, while our results are encouraging showing more rapid second intention wound healing in the PRP group compared to controls. There is no evidence to support the use of PRP in subdermal plexus skin flaps in cats. However, the use of PRP may aid in reducing edema of these flaps.Η δημιουργία δερματικών ελλειμμάτων στη γάτα οφείλεται συνήθως σε τραυματικές κακώσεις, ογκολογικές επεμβάσεις ή σε δερματικές λοιμώξεις. Τα δερματικά ελλείμματα στη γάτα μπορεί να επουλωθούν κατά πρώτο ή δεύτερο σκοπό, καθώς και με κρημνούς ή μοσχεύματα. Σκοπός της παρούσας διατριβής ήταν η μελέτη της επίδρασης που έχει η έγχυση πλάσματος πλούσιου σε αιμοπετάλια (PRP) σε ελλείμματα και σε κρημνούς του υποχοριοειδούς πλέγματος σε γάτες. Σε 8 γάτες, σχεδιάστηκαν 3 ελλείμματα 2Χ2 εκ. και ένας κρημνός με 2 εκ. πλάτος και 6 εκ. μήκος, εκατέρωθεν της μέσης ραχιαίας γραμμής, με τα ελλείμματα και τον κρημνό της μιας πλευράς να αποτελούν την ομάδα στην οποία έγινε έγχυση PRP, ενώ της άλλης πλευράς την ομάδα των μαρτύρων. Η εκτίμηση της επούλωσης των ελλειμάτων και των κρημνών βασίστηκε στην καθημερινή κλινική εξέταση, καθώς και στη μέτρηση της αιματικής ροής με LaserDoppler, στη φωτοπλανημετρία και στην ιστοπαθολογική εξέταση τις ημέρες 0, 7, 14, και 25, ενώ επιπλέον στα ελλείμματα πραγματοποιήθηκε και μέτρηση των μεταλλοπρωτεϊνασών (MMPs)-2 -9 και του ιστικού αναστολέα των μεταλλοπρωτεϊνασών (TIMP)-1 τις ημέρες 0, 14, και 25. Σύμφωνα με τα αποτελέσματα της μελέτης, ο μέσος χρόνος για την πλήρη κάλυψη των ελλειμμάτων με κοκκιώδη ιστό ήταν μικρότερος στα ελλείμματα-PRP (10,88 ± 1,26 ημέρες) σε σύγκριση με τα ελλείμματα-μάρτυρες (12,13 ± 1,28 ημέρες) [p=0,008].Το μέσο % ποσοστό συρρίκνωσης στα ελλείμματα-PRP ήταν αυξημένο σε σχέση με τα ελλείμματα-μάρτυρες (52,02% και 40,75%, αντίστοιχα)[p=0,002].
