Abstract. Background. Metabolic and hemodynamic factors concur to the development of diabetic nephropathy. Moreover, in diabetes, the presence of hypertension may accelerate the development of renal damage. Vascular Endothelial Growth Factor (VEGF) stimulates microvascular permeability, endothelium-dependent vasodilation and angiogenesis, and its synthesis is enhanced by hyperglycemia, Advanced Glycation End-products (AGEs), tissue hypoxia and hypertension. VEGF appears to play a central role in mediating diabetic vasculopathy, and although VEGF and its receptors are expressed at renal level, its action in renal pathophysiology is unknown.
