Evaluation of postoperative complications of mitral valve replacement versus mitral valve repair for mitral regurgitation

Cilj: Ispitati postoji li razlika postoperacijskih komplikacija liječenja mitralne regurgitacije metodama zamjene i rekonstrukcije mitralne valvule. Nacrt studije: Presječna studija s povijesnim podatcima. Ispitanici i metode: U studiju su uključeni bolesnici operirani na Zavodu za kardijalnu kirurgiju KBC-a Osijek u vremenskom razdoblju od 2008. do 2020. godine. Ispitanici su podijeljeni u dvije skupine. Prvu skupinu čine bolesnici operirani na Odjelu za kardijalnu kirurgiju metodom zamjene mitralne valvule, dok je druga skupina bolesnika operirana metodom rekonstrukcije mitralne valvule. Podatci su o bolesnicima prikupljeni iz povijesti bolesti i operacijskih lista. Rezultati: Od ukupno 142 bolesnika, 55,6 % su muškarci, a 44,4 % su žene. Fibrilacija atrija značajno je češća postoperacijska komplikacija nakon rekonstrukcije mitralne valvule, dok je kod ispitanika iz skupine zamjena prisutan postoperacijski infarkt. Rijetko se pojavljuju postoperacijski cerebrovaskularni incident i smrtni ishod. Infekcije se pojavljuju kod svakog četvrtog ispitanika. Dob i spol ispitanika nije značajno povezana s niti jednom komplikacijom. Postoje značajne razlike među ispitanicima u danima provedenim na JIL-u s obzirom na NYHA stupanj. Zaključak: Operacija je zamjene povezana s pojavom postoperacijskog infarkta, a kod rekonstrukcije češća fibrilacija atrija. Dob i spol nisu značajno povezani s postoperacijskim komplikacijama.Objectives: To examine whether there is a difference in postoperative complications of mitral regurgitation treatment by mitral valve replacement and reconstruction methods. Study design: Cross-sectional study with historical data Subjects and methods: The study included patients operated on at the Department of Cardiac Surgery, University Hospital Center Osijek in the period from 2008 to 2020. Subjects were divided into two groups. The first group of patients were operated on at the Department of Cardiac Surgery by mitral valve replacement, and the second group of patients underwent a surgery by mitral valve reconstruction. Data about patients was collected from the case history and operating lists. Results: Out of a total of 142 patients, 55.6 % were men and 44.4 % were women. Atrial fibrillation is a significantly more common postoperative complication after mitral valve reconstruction, while the subjects from the replacement group had a postoperative infarct. Postoperative cerebrovascular accident and death are rare. Infections occur with every fourth subject. The age and sex of the subjects were not significantly associated with any of the complications. There are significant differences among subjects in the days spent in the intensive care unit regarding NYHA status. Conclusion: Replacement surgery is associated with the occurrence of postoperative infarcts, and atrial fibrillation is more common in reconstruction.
