Pengaruh Penyakit Komorbid pada Pasien Terkonfirmasi COVID-19 Terhadap Hasil Pemeriksaan Laboratorium Penanda Inflamasi

Inflammatory reactions play an important role in the pathophysiology of COVID-19. Levels of D-dimer, procalcitonin (PCT) and C-reactive Protein (CRP) are known to increase in response to an inflammatory reaction. Comorbid COVID-19 patients are often associated with severe conditions and do not survive. In handling COVID-19 patients with comorbidities, it is necessary to recommend specific laboratory examinations to make the actions given more effective and efficient and reduce costs and treatment time. This study aims to determine the effect of comorbid on the results of laboratory examination of inflammatory markers. The study was conducted in June – August 2022 at Siloam Lippo Village Hospital. Subjects and laboratory results were presented descriptively. Test the effect of comorbid and inflammatory marker parameters using a statistical regression test (a=0.5%). Statistical tests showed no significant effect on comorbidity on laboratory results of inflammatory markers D-dimer (P= 0.467) and PCT (P= 0.834). Comorbidities significantly affected CRP examination results (P = 0.002). In this case, it is necessary to prioritize all comorbid COVID-19 patients to have an early CRP examination to treat rapidly decreasing oxygen saturation, deep vein thrombosis and pulmonary embolism and death.Reaksi inflamasi memainkan peran penting dalam patofisiologi COVID-19. Sitokin proinflamasi meningkat dalam dalam darah perifer pasien, demikian pula peningkatan penanda inflamasi seperti D-dimer, Procalcitonin (PCT) dan C-reactive protein (CRP). Pasien COVID-19 dengan komorbid sering dikaitkan dengan kondisi parah dan tidak selamat selama perawatan. Dalam penanganan pasien COVID-19 dengan komorbid diperlukan rekomendasi pemeriksaan laboratorium yang spesifik agar tindakan yang diberikan lebih efektif dan efisien sehingga biaya dan waktu perawatan dapat ditekan. Penelitian ini bertujuan untuk melihat pengaruh komorbid terhadap hasil pemeriksaan laboratorium penanda inflamasi. Penelitian dilakukan pada Juni – Agustus 2022 di RS Siloam Hospital Lippo Village. Sampel ditentukan berdasarkan kriteria inklusi. Pemeriksaan sampel dilakukan pada parameter penanda inflamasi D-dimer, CRP dan procalcitonin (PCT). Sampel dianalisa menggunakan alat Sysmex CS 1600 (D-dimer), Cobas C501 (CRP) dan Cobas E501 (PCT). Data komorbid pasien dan hasil pemeriksaan laboratorium disajikan secara deskriptif. Uji pengaruh komorbid dan parameter penanda inflamasi menggunakan uji statistik regresi linier (a=0,5%). Hasil uji statistik menunjukkan tidak ada pengaruh signifikan pada komorbid terhadap hasil laboratorium penanda inflamasi D-dimer (P= 0,467) dan PCT (P=0,834). Komorbid menunjukkan pengaruh yang signifikan pada hasil pemeriksaan CRP (P = 0,002). Dalam hal ini perlu diprioritaskan pada semua pasien COVID-19 dengan komorbid untuk dilakukan pemeriksaan CRP diawal untuk menangani dengan cepat menurunnya saturasi oksigen, thrombosis vena dalam dan emboli paru serta kematian
