The connection between aseptic inflammation, infectious and autoimmune factors, oxidative stress in pathogenesis of chronic prostatitis, chronic pelvic pain syndrome and lower urinary tract symptoms is considered. The limitations of the currently accepted prostatitis classification and new principles to overcome them are discussed. The algorithms of prostatitis diagnosis offered by the Russian Society of Urologists and the European Association of Urology are described.
