Ультразвуковая диагностика эндометрита: особенности морфологических типов в В-режиме

Aim. Despite significant advances in modern gynecology, endometritis is often unrecognized, being the main reason leading to persistent infertility or repeated pregnancy loss. There are 3 morphological types of chronic endometritis: hyperplastic, hypoplastic and mixed. The aim of the study was to develop ultrasound signs and hemodynamic features of various morphological types of chronic endometritis. Matherials and Methods. A retrospective analysis of 162 patients of reproductive age (22-50 years old) diagnosed with endometritis was carried out. The comparison group consisted of 96 women 17-42 years old without gynecological pathology (P > 0.05). The study was conducted in the proliferative phase of the cycle up to 10 days. The structure and echogenicity of the endometrium, the line of closure of the mucosal leaves, the gaping in the uterus, the state of the M-echo contour, the structure of the myometrium and the presence of acoustic effects were evaluated. Results. Depending on the morphotype of endometritis, echographic signs of endometritis are systematized, their frequency of occurrence is determined, and hemodynamic features are assessed. It was revealed that in the hypoplastic type, to which patients are related with the ratio of the endometrial volume to the volume of the uterus less than 2.5 %, there is a significant (P 0,05). Исследование проводилось в пролиферативную фазу цикла до 10 дня. Оценивалась структура и эхогенность эндометрия, линия смыкания листков слизистой, зияние в полости матки, состояние контура М-эха, структура миометрия и наличие акустических эффектов. Результаты. В зависимости от морфотипа эндометрита систематизированы эхографические признаки эндометрита, определена их частота встречаемости, а также оценены особенности гемодинамики. Выявлено, что при гипопластическом типе, к которому отнесены пациентки с отношением объема эндометрия к объему тела матки менее 2,5 %, наблюдается достоверное (P < 0,05) снижение ИАП, а также гиповаскуляризация матки. Наиболее выраженный полиморфизм признаков эндометрита в В-режиме отмечен при смешанном морфотипе, когда отношение объема эндометрия к объему тела матки было от 2,5 до 9,0 %. Выводы. Своевременная диагностика хронического эндометрита позволит обоснованно проводить лечение и восстановить репродуктивную функцию женщин
