ANALISIS RESIKO KECELAKAAN KERJA DENGAN MENGGUNAKAN METODE FTA(FAULT TREE ANALYSIS) DAN METODE FMEA (FAILURE MODE AND EFFECT ANALYSIS) DI PABRIK TAHU ADMA NU MALANG

Occupational health and safety (K3) is very important because it affects the company both directly and indirectly in the short and long term. Production started in the Karang Ploso area, but expanded to Dinoyo and surrounding areas as well. This factory carries out the production process for 7 days a week, namely Monday - Sunday and has 20 workers whose system is wholesale. Factory workers do not yet understand the importance of personal protective equipment (PPE) in their daily tasks. Humans, machines, materials/methods, and the work environment are all under the control of occupational safety and health (K3). In this study, a risk analysis of work accidents was carried out using the FTA and FMEA methods. In data processing using the FMEA method, namely by calculating the percentage of failures with a Pareto diagram. In the calculation using the Pareto diagram, the highest percentage of failure is slipping 38%. The fishbone diagram is to find out the cause and effect of the accident. In the RPN calculation, the highest value is slipping with a total of 120. In the FTA method, namely looking for failure trees in all work accidents, namely slipping, scratching tools, falling and being scalded with hot water. The proposed improvement is by analyzing using 5W + 1H, namely by providing a special place for water disposal so that the production floor is not getting slippery, facilitating complete PPE for workers and providing rules regarding work safety
