ASUHAN KEPERAWATAN PADA Tn.D DENGAN GANGGUAN PERSEPSI SENSORI HALUSINASI PENGLIHATAN di UPT BINA LARAS PASURUAN

Introduction : Hallucinations are disorders (perceptions) of the five senses in the absence of external stimuli that can include all sensing systems that occur at the time when the individual's consciousness is full / good. Hallucinations can be a reason why the client performs acts of violent behavior because of the voices that give him commands so that he is prone to perform non-adaptive behaviors. Violent behavior that arises in schizophrenic clients begins with the presence of feelings of worthlessness, fear and rejection by the environment so that the individual will get out of the way of interpersonal relationships with others. Purpose : This case report is to analyze the nursing care given to Mr.D with a diagnosis of sensory perception disorder: vision hallucinations at UPT Bina Laras Pasuruan Method: This research method is a case study (case report) Result: The results of the intervention were found that there had been no good changes in the client. The client still says hallucinations still appear when the client is aloof. Conclusion: The client is already willing to be invited to have a conversation, but is still reluctant to get together with many people. Thus, the client still feels his hallucinations when he is alone. Interventions given to clients should be carried out periodically so as to reduce the intensity of hallucinatory event
