Biplane double-supported screw fixation (BDSF) at femoral neck fractures // Двупланова двуопориа винтова фиксация (BDSF) при фрактури на бедрената шийка

[EN] Osteosynthesis of femoral neck fractures is related to 20-46% poor results. The method, recently introduced and clinically applied as of 2007, known as Filipov's method for biplane double-supported screw fixation (BDSF), provides better stability, buttressing two out of three medially diverging cannulated screws on the inferior cortex with additional support of the distal screw on the posterior neck cortex. The two calcar-buttressed screws are oriented in different coronal inclinations and intended to provide constant fixation strength during various physical activities. This study evaluates retrospectively the results from the first 5-year period of BDSF clinical application. Results in 222 patients, 207 of whom with displaced fractures (15 Garden III and 192 Garden IV), 42 males and 165 females. The median age was 76.3 years. The mean follow-up period was 29.6 months (range 12-78). The patients aged <65 we divided in separate group and all patients aged >66 years we further divided into age groups of five years each. Results: Bone union occurred in 96.61% (200/207). Nonunion occurred in 7 patients (3.38%), including fixation failure in 5 cases (2.41%), pseudoarthrosis in one patient (0.48%) and nonunion with AVN in one case (0.48%). Avascular necrosis developed in 25 patients (12.07%), 2/42 males (4.76%) and 23/165 females (13.93%). Harris hip score was higher for males (89.3±18.1) versus females (85.4±19.1), p=0.07. Age of patients declaring good mobility (75.4± 10.2) was lower compared to patients with poor mobility (80.8±5.9). Age of patients declaring easy putting on shoes (75.1±10.1) or with less than 1 comorbidity (55.8±3.4) was significantly lower compared to patients with difficult putting on shoes (81.3±6.7) or with more than 2 comorbidities (76.7±9.5), respectively (p<0.01 for all cprs.). /////// /////// [BG] Остеосинтезата при фрактури на бедрената шийка е свързана с 20-46% лоши резултати. Наскоро въведеният метод на Филипов Двупланова двуопорна винтова фиксация (BDSF) осигурява увеличена стабилност, опирайки два от трите медиално дивергиращи канюлирани винта върху дисталния кортекс, с допълнителна опора върху дорзалния кортекс на шийката. Двата калкарно- опряни винта са с различни коронарни инклинации, за да осигурят константна здравина на фиксацията при различните физически активности на пациента. Студията оценява ретроспективно резултатите от първите 5 години на клинично приложение на BDSF. Резултатите при 222 пациенти, от които 207 (76.3±9.8 години, мъже 75.7±10.3, жени 76.4±9.8) с разместени Garden III— IV фрактури бяха анализирани при 29.6±16.8-месеца проследяване.
