Pituitary apoplexy (PA) is a rare, potentially life-threatening clinical syndrome resulting from acute infarction or hemorrhage of the pituitary gland1. Most commonly a sequela of pituitary adenoma or pregnancy, PAs have the highest incidence in large macroadenomas2,3. While rapid intervention is critical to prevent permanent ophthalmic damage, neurosurgical versus conservative management of PA continues to be controversial4. We report a case of giant prolactinoma with secondary PA, with rapid recovery from severe vision loss through cabergoline therapy alone
