Medical records at puskesmas health service facilities must be kept at least for a period of 2 years from the last date the patient was treated. After this period of time, medical records can be retained and destroyed. The Wonopringgo Health Center has implemented retention in 2017, but so far the retention has not been re-implemented so that active and inactive medical records are still used as one place since 2017. This causes the storage shelves to become full and hampers the performance of officers in managing medical records. The purpose of this study was to determine the implementation of medical record retention according to aspects of 5M management (man, method, material, machine, money) at the Wonopringgo Health Center.This type of research uses descriptive qualitative research. Methods of data collection using interviews and observation. Data processing techniques using data analysis techniques with fishbone analysis.The results showed that the implementation of medical record retention from the man aspect, namely the absence of human resources officers with medical record education backgrounds and the unfulfilled human resources of the retention and destruction team, from the method aspect, namely there was no Archive Retention Schedule, from the machine aspect, namely the absence of annihilation tools and from money aspect, namely the unavailability of a special budget for HR training, as well as the implementation of retention and destruction. Suggestions for this research are that Puskesmas need to form a retention and destruction team, make a schedule for retention of medical records, include officers for training and plan a special budget for retention and destruction
