Lichen hypertrophicus (LPH) is a variant of lichen planus (LP) generally limited to lower extremities. Isolated involvement of the male external genitalia is rare. Although a standard accepted treatment is still to be determined, the treatment for LP consists, basically, of topical or systemic steroids. Because of the potential side effects of repeated topical application of potent corticosteroids, safer therapeutic options are required, especially in the treatment of genital lesions. In contrast to topical steroids, pimecrolimus (P) does not cause skin atrophy with its prolonged use. We report a man suffering from LPH involving the glans penis, successfully treated with topical P 1 % cream, twice daily
