Development of artificial intelligence algorithm for detection of intracranial hemorrhages post - traumatic brain injuries and its potential benefits onto SUS Fácil.

The study and use of Artificial Intelligence (AI) in medicine has grown exponentially. Auxiliary tools for identifying potential threats to life and prioritizing cases could be beneficial in the management of Traumatic Brain Injuries (TBIs) within the scope of the Unified Health System (SUS). These are public health problems, often resulting in Intracranial Hemorrhages (ICH) with high morbidity and mortality and socioeconomic costs, especially when not treated in a timely manner. In Minas Gerais, when they focus on cities with low hospital complexity, without neurosurgeons, radiologists, or computed tomographies (CTs), they need to transfer their patients, using the SUS FÁCIL bed regulation platform, through a hierarchical mechanism. The delay in the identification and registration of serious cases at the origin, their analysis by regulators in the SF regulation centers, could be alleviated with the potential insertion of an AI algorithm capable of automatically recognizing HICs in CTs at the origin sites, thus adding agility for the detection and prioritization of life-threatening cases. This work proposes the development of an ICH identification algorithm in CT and the analysis of its potential benefits in the context of SUS FÁCIL. After collecting and processing data sets images containing normal and HIC skull CTs, Orange® Software was used for training, validation and testing of their recognition by Artificial Neural Network (ANN) models, Support Vector Machines (SVM) and K – Nearest-Neighbors (KNN). The ANN model presented slightly better results than the other models in the Training / Validation and Testing stages (AUC = 1,000, CA = 0.998, F1 = 0.998, Accuracy = 0.998 and Recall = 0.998, and AUC = 0.987, CA = 0.930, F1 = 0.930, Precision = 0.931 and Recall = 0.930 respectively) achieving the proposed objective for the set of images used. However, the formulation of more robust datasets becomes necessary for their practical use and their improvement and use should be encouraged.Dissertação (Mestrado)O estudo e utilização da Inteligência Artificial (IA) na medicina têm crescimento exponencial. Ferramentas auxiliares à identificação de potenciais ameaças à vida e de priorização de casos poderiam ser benéficas na condução de Traumas Cranioencefálicos (TCEs) no âmbito do Sistema Único de Saúde (SUS). Estes são problemas de saúde pública, frequentemente resultando em Hemorragias Intracranianas (HICs) de elevada morbimortalidade e custos socioeconômicos, sobretudo quando não tratadas em tempo hábil. Em Minas Gerais, quando incidem em municípios de baixa complexidade hospitalar, sem neurocirurgiões, radiologistas ou tomografias computadorizadas (TCs), necessitam por mecanismo de hierarquização transferir seus pacientes usando-se a plataforma de regulação de leitos SUS FÁCIL. O retardo da identificação e cadastro dos casos graves na origem, da sua análise por médicos reguladores nas centrais de regulação SF, poderiam ser amenizados com a potencial inserção de um algoritmo de IA capaz de reconhecer automaticamente HICs em TCs nos locais de origem, agregando assim agilidade para a detecção e priorização de casos ameaçadores a vida. Este trabalho propõe o desenvolvimento de um algoritmo de identificação de HICs em TCs e a análise dos seus potenciais benefícios no contexto SUS FÁCIL. Após a coleta e processamento de imagens de datasets contendo TCs de crânio normais e com HICs, utilizou-se o Software Orange® para treinamento, validação e teste do seu reconhecimento por modelos de Redes Neurais Artificiais (ANN), Máquinas de Vetores de Suporte (SVM), Florestas Alelatórias (Random Forests) e K – Nearest- Neighbors (KNN). O modelo de ANN apresentou resultados ligeiramente superiores aos demais modelos nas etapas de Treinamento / Validação e Teste (AUC = 1.000, CA = 0.998, F1 = 0.998, Precisão = 0.998 e Recall = 0.998, e AUC = 0.987, CA= 0.930, F1= 0.930, Precision = 0.931 e Recall = 0.930 respectivamente) conseguindo atingir o objetivo proposto para o conjunto de imagens utilizadas. Contudo formulação de datasets mais robustos tornam-se necessários para seu uso prático e seu aperfeiçoamento e emprego devem ser incentivados
