Purpose of the Project: The purpose of the project was to test a quality improvement (QI) approach consisting of a multimedia education/accountability bundle on a sample of prediabetic patients in a primary care setting. Methodology: The intervention introduced the use of tracking apps, diabetes prevention bundle and telephone check-ins by primary care staff over the course of 9 to 12 weeks. Results: As expected all participants saw positive change with more frequent tracking predicting greater decrease in the three measured variables. Diet tracking provided the most significant weight, BMI and A1c changes.
