Coronoid process hyperplasia (CPH) is a rare disease characterized by the abnormal enlargement of the mandibular coronoid process. The most referred symptom is the progressive reduction of mouth opening due to the coronoid impingement on the zygomatic bone. This paper aims to describe a case of bilateral CPH treated with a bilateral intraoral endoscopically assisted coronoidectomy adopting in-house CAD/CAM technology. In May 2021, an 18-year-old man patient came to the authors' observation referring to a worsening limitation of the maximum mouth opening (MMO) and a CT scan revealing bilateral coronoid hyperplasia.
