Investigação celular e molecular do Osteossarcoma primário e metastático

Objectives: To identify biological and molecular alterations in the primary and metastatic osteosarcoma (OS). Methodology: 1) All specimens obtained from patients admitted at the Pediatric Oncology Institute with OS as suspect, between 2017 and 2019, were immediately processed for cell culture. All primary cell cultures were recorded by photomicrographs and cryopreserved at the Institutional Biobank. 2) The nuclei isolated from OS bulk were loaded into the Chromium controller and sequenced in the NovaSeq by Illumina®. 3) The sequencing of 31 genes, investigated regarding their expression, was performed in 32 paired specimens (primary and metastatic tumors), obtained from 16 patients, of which 11 were investigated regarding gene expression. The library was prepared using the Ion Chef™ Instrument and the sequencing using the Ion GeneStudio S5. 4)The expression of 32 genes were investigated in 80 paired specimens, including primary tumors (pre- and post-chemotherapy) and metastases, obtained from 33 patients. The quantitative real-time PCR (qRT-PCR) was performed, using TaqMan probes. Results and Conclusion: 1) Thirty-one paired primary cell cultures, obtained from different sites and moments of 13 OS patients, were established and cryopreserved at the Institutional Biobank. These paired primary cell cultures could be useful as new model of study to better understanding of tumorigenesis, treatment response and progression in OS. 2) The single nuclei transcriptome from OS bulk has not been successful, due to the difficulty in obtaining integrate RNA after nuclei isolation. 3) In total, 217 variants were identified, 145 unknown effect, 40 synonymous, 29 missense, 2 non-frameshift deletions and 1 not identified effect. The profile regarding the variant’s presence was different in some cases, when comparing the primary and metastatic tumor from the same patient. 4) The metastasis specimens presented higher expression of ABCC1 e ABCC4 and lower expression of ABCC10, ERCC2, MSH2, SLC22A1, SOD1 and TOP2A comparing to the primary tumors. The high expression of ERCC1 and TOP2A, and the low expression of ABCC3, FASLG and SLC22A1 were associated with worse treatment response. The molecular findings may, in the future, contribute to clinical management, as prognostic factors as well as possible therapeutic targets.Objetivos: Identificar alterações biológicas e moleculares no osteosarcoma (OS) primário e metastático. Métodos: 1) Todas as amostras provenientes de pacientes admitidos no Instituto de Oncologia Pediátrica com suspeita de OS, entre 2017 e 2019, foram processadas imediatamente para cultivo celular. Todas as culturas primárias obtidas foram registradas por fotomicrografias e criopreservadas no Biobanco Institucional. 2) Os núcleos das células da massa tumoral do OS foram isolados, carregados no Chromium controller e sequenciados no NovaSeq da Illumina®. 3) O sequenciamento de 31 genes foi realizado em 32 amostras pareadas (tumores primários e metastáticos), provenientes de 16 pacientes, dos quais 11 foram investigados em relação à expressão gênica. O preparo da biblioteca foi realizado no Ion Chef™ Instrument e o sequenciamento no Ion GeneStudio S5. 4) As expressões de 32 genes foram investigadas em 80 amostras pareadas, incluindo tumores primários (pré e pós-quimioterapia) e metástases, provenientes de 33 pacientes. Então, foi realizado o PCR quantitativo em tempo real (qRT-PCR), utilizando sondas TaqMan. Resultados e Conclusão: 1) Trinta e uma culturas celulares primárias pareadas, provenientes de diferentes sítios e momentos da doença de 13 pacientes com OS, foram estabelecidas e criopreservadas no Biobanco Institucional. As culturas primárias estabelecidas podem ser úteis como novo modelo de estudo para melhor compreender a tumorigênese, resposta ao tratamento e progressão em OS. 2) Até o momento, não foi obtido sucesso no transcriptoma de núcleo único de células da massa tumoral do OS, devido a dificuldade em se obter RNA íntegro após o isolamento dos núcleos. 3) No total, foram identificadas 217 variantes, sendo 145 de efeito desconhecido, 40 sinônimas, 29 missense, 2 deleções não-frameshift e 1 de efeito não identificado. O perfil de presença das variantes foi diferente em alguns casos, quando comparados os tumores primário e metastático do mesmo paciente. 4) As metástases de OS apresentaram alta expressão de ABCC1 e ABCC4 e baixa expressão de ABCC10, ERCC2, MSH2, SLC22A1, SOD1 e TOP2A, em relação ao tumor primário. A alta expressão de ERCC1 e TOP2A, e a baixa expressão de ABCC3, FASLG e SLC22A1 foram associadas a pior resposta ao tratamento.
