Acurácia dos testes clínicos e ressonância magnética no diagnóstico das lesões do tendão supraespinal em pacientes submetidos à artroscopia cirúrgica do ombro

Introduction: The shoulder clinical tests and magnetic resonance imaging are frequently performed in patients undergoing arthroscopic treatment of rotator cuff tears; however, the ability of these clinical tests to detect supraspinatus tendon lesions is a matter of debate in previous studies. Objective: To determine the diagnostic accuracy of clinical tests and magnetic resonance for the detection of the supraspinatus tendon lesions in patients undergoing shoulder arthroscopy. Methods: A total of 199 consecutive patients were prospectively evaluated in this multicenter study from April 2017 to April 2019, and eleven clinical tests and magnetic resonance imaging were compared with the results of the arthroscopy. We assessed the sensitivity, specificity, accuracy, the positive and negative predictive values, diagnostic odds ratio (DOR), and the positive and negative likelihood ratio for all tests. Results: For overall tears, the Jobe’s test showed the highest sensitivity (0.88) and DOR (29.96) and the weakness in the Jobe’s and full can tests presented the best positive predictive value and specificity. For the location of the lesion, the full can test showed the highest sensitivity in the anterior tears, and the Jobe’s test in the middle third tears. There were greater specificity and Positive Predictive Value on the combination of Jobe, full can and Hawkins tests than the same isolated tests. Magnetic resonance imaging showed high sensitivity (0.99) and specificity (0.98) for full-thickness tears. Conclusion: The Jobe’s test showed the highest sensitivity and the combination of the Jobe, full can and Hawkins tests proved to be more specific for the diagnosis of supraspinatus tears. Magnetic resonance imaging demonstrated high diagnostic accuracy.Introdução: Os testes clínicos e a ressonância magnética do ombro são realizados com frequência nos pacientes submetidos ao tratamento artroscópico das lesões do manguito rotador; no entanto, a capacidade destes testes clínicos de detectar as roturas do tendão supraespinal é tema de debate na literatura. Objetivo: Avaliar a acurácia dos testes clínicos e da ressonância magnética no diagnóstico das lesões do supraespinal em pacientes submetidos a artroscopia do ombro. Métodos: Um total de 199 pacientes consecutivos foram avaliados prospectivamente neste estudo multicêntrico, entre abril de 2017 e abril de 2019, e onze testes clínicos e ressonância magnética foram comparados com os resultados da artroscopia. Foi avaliado a sensibilidade, a especificidade, a acurácia, os valores preditivos positivos e negativos, a razão de chances de diagnóstico (DOR) e a razão de verossimilhança positiva e negativa para todos os testes. Resultados: Para as roturas, o teste de Jobe apresentou maior sensibilidade (88%) e melhor performance (DOR = 29,96), e a perda de força nos testes de Jobe e full can apresentou maior valor preditivo positivo e especificidade. Para localização da lesão, o full can teste apresentou maior sensibilidade nas lesões anteriores, e o teste de Jobe nas lesões do terço médio. Na combinação dos testes Jobe, full can e Hawkins houve maior especificidade e Valor Preditivo Positivo do que os mesmos testes isolados. A ressonância magnética apresentou alta sensibilidade (99%) e especificidade (98%) para as roturas totais. Conclusão: O teste de Jobe apresentou maior sensibilidade e a combinação dos testes de Jobe, full can e Hawkins foi mais específica no diagnóstico das roturas do supraespinal.
