Adipokines, cytokines and biomarkers of insulin resistance in women with postmenopausal breast cancer

Introduction: Dysregulation of adipokines, hormones secreted from adipose tissue, has been reported in various solid tumors, including postmenopausal breast cancer (PBC). While classic adipokines, such as leptin and adiponectin, have been associated with BC in many studies, little is known about novel adipokines.Aim: The aim of the present doctoral thesis was to investigate the association of classic adipokines, especially the newly discovered omentin-1, which is an anti-inflammatory and proapoptotic adipokine, with PBC taking into consideration clinicopathologic features, inflammatory parameters, biomarkers of insulin resistance, tumor markers, anthropometric, dietary and cardiometabolic parameters as well as their diagnostic potential.Materials and methods: In this case-control study serum leptin, adiponectin and omentin-1, inflammatory parameters (TNFa, IL-6 and hsCRP), biomarkers of insulin resistance (insulin, HOMA-IR index, presence and grade of metabolic syndrome), tumor markers (CA15-3 and CEA), metabolic (glucose and serum lipids), anthropometric (BMI, waist circumference and fat mass), blood pressure (systolic, diastolic and mean BP) parameters, the Mediterranean diet (MedDiet) score, and cardiovascular risk (Framingham risk score) were determined in 103 postmenopausal women with pathologically confirmed incident invasive BC who had not received any type of therapy, 103 controls matched on age and time to the diagnosis, 51 patients with benign breast lesions (BBL), and 50 obese postmenopausal women without any kind of malignancy of similar age.Results: The mean serum leptin of cases with PBC was similar to the corresponding levels of controls (p=0.7) or women with BBL (p=0.6), but was significantly lower than in participants with obesity (p<0.001). The mean serum adiponectin was significantly lower in cases than in controls (p=0.04) and was similar to the corresponding levels of women with BBL (p=0.30) or participants with obesity (p=0.44). The mean serum omentin-1 was significantly lower in cases than in controls or women with BBL (p<0.001), but was similar to the corresponding levels of participants with obesity (p=0.21). In cases with PBC, omentin-1 was inversely associated with anthropometric, tumor, metabolic and inflammatory biomarkers, cancer stage, and the number of infiltrated lymph nodes (p<0.05). In all study participants, circulating omentin-1 was negatively correlated with cardiometabolic risk factors, such as Framingham risk score, and was positively correlated with MedDiet score and HDL-C. Cases with PBC, tended to present a more frequent history and a higher grade of metabolic syndrome than in controls, but a less frequent history and a similar grade of metabolic syndrome than in participants with obesity. Lower circulating omentin-1 was independently associated with PBC occurrence above and beyond known risk factors, as well as metabolic and inflammatory parameters. According to the ROC curve analysis, the overall diagnostic performance of omentin-1 was similar to that of CA15-3 in PBC diagnosis, but with a poorer discriminative ability in postmenopausal women with obesity. Additionally, the diagnostic performance of omentin-1 was better than the diagnostic performance of CEA, adiponectin and leptin in the diagnosis of PBC.Conclusions: Serum omentin-1 may be a biomarker at the intersection of PBC and cardiometabolic risk in postmenopausal women. Circulating omentin-1 could be modulated by the adoption of the anti-inflammatory and antioxidant MedDiet. Low circulating omentin-1 may signal lymph node invasion, negative hormonal receptor status and increased cardiometabolic risk in postmenopausal women with BC. However, further mechanistic and large multicentric prospective and longitudinal studies are required to elucidate the ontological role of the newly discovered omentin-1 in BC and CVD risks, as well as its diagnostic and prognostic ability and its therapeutic potential.Εισαγωγή: Η απορρύθμιση στην έκφραση των λιποκινών, ορμονών του λιπώδους ιστού, έχει αναφερθεί σε πολλές κακοήθειες συμπαγών οργάνων, συμπεριλαμβανομένου του μετεμμηνοπαυσιακού καρκίνου του μαστού (ΚΜ). Ενώ για τις κλασικές λιποκίνες, όπως η λεπτίνη και η αδιπονεκτίνη, έχουν γίνει αρκετές μελέτες αναφορικά με τη συσχέτισή τους με τον ΚΜ λιγότερα ερευνητικά δεδομένα είναι διαθέσιμα για τις νεότερες λιποκίνες.Σκοπός: Η διερεύνηση της σχέσης των λιποκινών και ειδικότερα της νεότερης λιποκίνης ομεντίνης-1, μιας αντιφλεγμονώδους και προαποπτωτικής λιποκίνης, με τον μετεμμηνοπαυσιακό ΚΜ, λαμβάνοντας υπόψιν μεταξύ άλλων κλινικά και παθολογοανατομικά χαρακτηριστικά, βιοδείκτες φλεγμονής, δείκτες ινσουλινοαντίστασης, καρκινικούς δείκτες καθώς και σωματομετρικές, διατροφικές και καρδιομεταβολικές παραμέτρους.Υλικό και μέθοδοι: Σε αυτή τη διασκοπική μελέτη που πραγματοποιήθηκε στο πλαίσιο εκπόνησης διδακτορικής διατριβής προσδιορίστηκαν οι κλασικές λιποκίνες (λεπτίνη και αδιπονεκτίνη) και η ομεντίνη-1 ορού, βιοδείκτες φλεγμονής (hsCRP, TNFα και IL-6), δείκτες ινσουλινοαντίστασης (ινσουλίνη, δείκτης ινσουλινοαντίστασης HOMA-IR, παρουσία και βαρύτητα του μεταβολικού συνδρόμου), καρκινικοί δείκτες (CA15-3 και CEA), μεταβολικές (γλυκόζη και λιπίδια) και σωματομετρικές (Δείκτης Μάζας Σώματος, περίμετρος μέσης και λιπώδης μάζα) παράμετροι, δεδομένα αρτηριακής πίεσης (συστολική, διαστολική και μέση αρτηριακή πίεση), ο βαθμός συμμόρφωσης στη μεσογειακή δίαιτα (MedDiet score) και ο 10ετής καρδιαγγειακός κίνδυνος (Framingham Risk Score) σε 103 μετεμμηνοπαυσιακές ασθενείς με πρωτοδιαγνωσμένο διηθητικό ΚΜ πριν την οποιαδήποτε θεραπευτική παρέμβαση, 103 υγιείς μάρτυρες εξομοιωμένες ως προς την ηλικία και τον χρόνο ένταξης στη μελέτη, 51 συμμετέχουσες με ινοκυστική μαστοπάθεια (ΙΚΜ) και 50 συμμετέχουσες με παχυσαρκία χωρίς κάποια διαγνωσμένη κακοήθη νόσο, παρόμοιας ηλικίας με τις ασθενείς.Αποτελέσματα: Τα επίπεδα της λεπτίνης στον ορό των ασθενών ήταν παρόμοια με αυτά των μαρτύρων (p=0.7) ή των μετεμμηνοπαυσιακών γυναικών με ΙΚΜ (p=0.6), ενώ ήταν σημαντικά μειωμένα σε σχέση με αυτά των μετεμμηνοπαυσιακών γυναικών με παχυσαρκία (p<0.001).
