Τhe role of Rotational Thromboelastometry (ROTEM) in prediction of venous thromboembolism and transfusion requirements following major orthopedic surgeries

Introduction: Trauma induced coagulopathy has been extensively investigated in the multi-trauma setting, but only sparsely following moderate orthopedic trauma. There are no reliable laboratory assays to identify patients at risk for venous thromboembolic (VTE) events after major orthopaedic surgery. Hip fracture surgeries are associated with considerable blood loss, while the perioperative coagulopathy is associated with the bleeding risk of these patients. The frequency of red blood cell (RBC) transfusions is high in total joint arthroplasties, and the hemorrhagic risk is associated with both surgery- and patient-related factors.Purpose: The purpose of this study is to evaluate changes in the hemostatic profile of patients with hip fractures, using rotational thromboelastometry (ROTEM). Also, we aimed to evaluate whether ROTEM findings are associated with the presence or development of symptomatic VTE after surgery for hip fractures and identify which ROTEM parameters were the most accurate in terms of detecting the association of hypercoagulability with symptomatic VTE. Moreover, the purpose of the study was to evaluate the ability of rotational thromboelastometry to detect patients at high risk for excessive bleeding and increased transfusion requirements in hip fracture surgeries, and to assess the ability of rotational thromboelastometry to identify patients at high risk for transfusion and excessive bleeding in total joint arthroplasties. Methods: For the evaluation of the changes in the hemostatic profile of patients with hip fractures, 198 patients with hip fractures who underwent surgery were included in the study. A matched group of 52 healthy individuals was also enrolled. Demographics, conventional laboratory assays and ROTEM parameters were recorded and compared between patients and healthy adults. The preoperative and postoperative ROTEM values of fractured patients were also compared. For the evaluation of the ability of ROTEM to detect the association of hypercoagulability with symptomatic VTE, a prospective observational study was conducted in which 354 patients with femoral neck and peritrochanteric fractures who underwent hip hemiarthoplasty or cephallomedullary nailing were assessed for eligibility. Of those, 99% (349 of 354) were considered eligible for the study while 1% (3 of 354) was excluded due to coagulation disorders and another 1% (2 of 354) because they died before the postoperative ROTEM analysis; further 4% (13 of 354) of the patients were lost prior to the minimum study follow-up of 3 months, leaving 95% (336 of 354) for analysis. A ROTEM analysis was performed in all patients at the time of their admission to the hospital, within hours from the injury, and on the second postoperative day. The patients were monitored for the development of indicative symptoms for VTE and the gold standard tests for diagnosis of VTE such as CT pulmonary angiography or vascular ultrasound were selectively performed only to symptomatic patients and not routinely to all patients. To determine whether ROTEM findings were associated with the presence or development of symptomatic VTE, ROTEM parameters were compared between patients with and without symptomatic VTE. To determine which ROTEM parameters were the most accurate in terms of detecting the association of hypercoagulability with symptomatic VTE, the area under curve (AUC) for certain cut off values of ROTEM parameters were calculated. For the evaluation of the ability of rotational thromboelastometry to detect patients at high risk for excessive bleeding and increased transfusion requirements in hip fracture surgeries a prospective observational study of 221 patients who underwent hip fracture surgeries was conducted. ROTEM analysis was performed preoperatively and immediately postoperatively. Blood loss parameters including blood loss volume, number of transfused red blood cells (RBC) units and drop in hemoglobin levels were recorded. ROTEM parameters were compared between patients with and without excessive bleeding, and between patients with and without increased transfusion requirements (i.e., ≥ 2 RBC units). For the evaluation of the ability of rotational thromboelastometry to identify patients at high risk for transfusion and excessive bleeding in total joint arthroplasties a prospective observational study was conducted including 206 patients who underwent total knee or hip arthroplasties. Assessment of the coagulation status was performed preoperatively and immediately postoperatively using ROTEM analysis and conventional coagulation tests. The number of RBC transfusions and the postoperative hemoglobin drop were recorded. ROTEM findings were compared between transfused and non-transfused patients, and also between patients with and without excessive bleeding. Results: In the part of the study regarding the evaluation of the changes in the hemostatic profile of patients with hip fractures, the conventional coagulation assays were similar for the 2 groups. However, several ROTEM parameters including EXTEM MCF (p<0.001), EXTEM alpha angle (p<0.001), INTEM MCF (p<0.001), INTEM A10 (p<0.001) and INTEM alpha angle (p<0.001) significantly differed between the 2 groups indicating a higher coagulation potential following hip fractures. Also, fractured patients had significantly lower INTEM and EXTEM CT values (p=0.008 and p=0.012 respectively), and significantly lower INTEM and EXTEM CFT values (p<0.001). Adjusted analysis for confounders further confirmed the direct relationship between hip fracture and higher coagulation activity. Last, INTEM CT and CFT significantly decreased (p=0.008 and p<0.001 respectively), while INTEM MCF, A10 and alpha angle significantly increased (p<0.001) postoperatively, indicating that surgery further increases the coagulation potential. In the part of the study regarding evaluation of the ability of ROTEM to detect the association of hypercoagulability with symptomatic VTE we found several abnormal ROTEM values to be associated with the presence or development of symptomatic VTE. The preoperative maximum clot firmness (MCF) was higher in patients with clinically evident VTE than in patients without these complications (median, interquartile range [IQR]: 70, 68-71 vs 65, 61-68; p<0.001). The preoperative clot formation time (CFT) was lower in patients with clinically evident VTE than those without clinically evident VTE (median, IQR: 61, 58-65 vs 70. 67-74; p<0.001), while also the postoperative CFT was lower in patients with clinically evident VTE than those without these complications (median, IQR: 52, 49-59 vs 62, 57-68; p<0.001). Last, preoperative CFT demonstrated the best performance for detecting the association of hypercoagulability with symptomatic VTE (AUC, 0.89; 95% CI, 0.81-0.97) with 81% (95% CI:48%-97%) sensitivity and 86% (95% CI:81%-89%) specificity for CFT value ≤ 65 seconds. In the part of the study regarding the evaluation of the ability of rotational thromboelastometry to detect patients at high risk for excessive bleeding and increased transfusion requirements in hip fracture surgeries, the postoperative FIBTEM MCF value ≤ 15 mm had 66.6% (95% CI: 59.7%-74.1%) sensitivity and 92.0% (95% CI: 80.7%-97.7%) specificity to prognose excessive bleeding, and preoperative FIBTEM MCF value ≤ 15 mm had 80.4% (95% CI: 73.5%-86.2%) sensitivity and 91.2 % (95% CI: 80.7%-97.0%) specificity to prognose increased transfusion requirements. Preoperative FIBTEM MCF ≤11 mm and postoperative FIBTEM MCF ≤15 mm were associated with considerably increased risks of excessive bleeding (OR: 44.8, 95% CI: 16.5–121.3, p<0.001; and OR: 23.0, 95% CI: 7.8–67.0, p<0.001, respectively). In the part of the study regarding the evaluation of the ability of rotational thromboelastometry to identify patients at high risk for transfusion and excessive bleeding in total joint arthroplasties, higher values of postoperative FIBTEM MCF were associated with lower risks of transfusion (OR: 0.66, 95% CI: 0.57–0.78, p<0.001) and excessive bleeding (OR: 0.58, 95% CI: 0.36–0.94, p=0.028). A postoperative FIBTEM MCF value ≤ 10 mm had 80.1% (95% CI: 73.1%–85.9%) sensitivity with 75.5% (95% CI: 60.4%–87.1%) specificity to predict transfusion requirements, and 70.5% (95% CI: 63.6%–76.8%) sensitivity with 88.8% (95% CI: 51.7%–99.7%) specificity to predict excessive bleeding. The estimated average probability of transfusion in patients with FIBTEM MCF values 0–4 mm is 86.3%.Conclusion: A higher coagulation activity following hip fractures and surgical treatment can be detected by ROTEM shortly after the injury, even when this is undetectable by conventional coagulation assays. ROTEM’s performance in this preliminary study was promising in terms of its association with symptomatic VTE. ROTEM demonstrated a high accuracy in detecting the level of hypercoagulability that is associated with symptomatic VTE. However, until its performance is validated in a study that applies a diagnostic gold standard (such as venography, duplex/Doppler, or chest CT) in all patients having ROTEM to confirm its performance, ROTEM should not be used as a regular part of clinical practice. Also, ROTEM parameters demonstrated high prognostic accuracy for excessive bleeding and increased transfusion requirements after surgery for hip fractures, and high predictive ability for transfusion and excessive bleeding after total hip and knew arthroplasties. This can enable implementation of blood sparing strategies in high risk patients, while blood banks could be better prepared to ensure adequate blood supply.Εισαγωγή: Οι διαταραχές της πήξης λόγω τραύματος έχουν διερευνηθεί εκτενώς στα πλαίσια του μείζονος τραύματος, αλλά ελάχιστα στα πλαίσια του ενδιάμεσου βαθμού σοβαρότητας του ορθοπεδικού τραύματος. Δεν υπάρχουν αξιόπιστες εργαστηριακές δοκιμασίες για την πρόβλεψη των φλεβικών θρομβοεμβολικών επεισοδίων (ΦΘΕ) μετά από μείζονες ορθοπαιδικές χειρουργικές επεμβάσεις. Τέλος, η συχνότητα μετάγγισης συμπυκνωμένων ερυθρών αιμοσφαιρίων (ΣΕΑ) είναι υψηλή σε ολικές αρθροπλαστικές ισχίου και γόνατος, ενώ ο αιμορραγικός κίνδυνος συνδέεται τόσο με το χειρουργείο, όσο και με παράγοντες που σχετίζονται με την αιμοστατική ικανότητα του ασθενούς.Σκοπός: Ο σκοπός της παρούσας μελέτης ήταν η αξιολόγηση των μεταβολών στο αιμοστατικό προφίλ των ασθενών με κατάγματα ισχίου, χρησιμοποιώντας την δοκιμασία της περιστροφικής θρομβοελαστομετρίας (rotational thromboelastometry- ROTEM). Επίσης, ήταν η αξιολόγηση της ικανότητας της περιστροφικής θρομβοελαστομετρίας στην πρόβλεψη θρομβοεμβολικών επιπλοκών μετά από χειρουργικές επεμβάσεις αποκατάστασης καταγμάτων ισχίου, η αξιολόγηση της ικανότητα της περιστροφικής θρομβοελαστομετρίας να ανιχνεύσει ασθενείς υψηλού κινδύνου για μείζονα αιμορραγία και αυξημένες ανάγκες μετάγγισης μετά από επεμβάσεις αποκατάστασης καταγμάτων ισχίου, και τέλος η αξιολόγηση της ικανότητας της περιστροφικής θρομβοελαστομετρίας να ανιχνεύσει ασθενείς υψηλού κινδύνου για μετάγγιση και αυξημένη αιμορραγία μετά από ολικές αρθροπλαστικές ισχίου και γόνατος. Υλικό και Μέθοδος: Για το αντίστοιχο μέρος της μελέτης που αφορούσε την αξιολόγηση των μεταβολών στο αιμοστατικό προφίλ των ασθενών με κατάγματα ισχίου συμπεριλήφθηκαν 175 ασθενείς που υποβλήθηκαν σε χειρουργική επέμβαση αποκατάστασης καταγμάτων ισχίου. Μια σταθισμένη ομάδα 43 υγιών μαρτύρων συμπεριλήφθηκε ως ομάδα ελέγχου. Τα δημογραφικά δεδομένα, οι συμβατικές εργαστηριακές δοκιμασίες και οι παράμετροι της δοκιμασίας ROTEM συγκρίθηκαν μεταξύ των ασθενών και των υγιών μαρτύρων. Για το αντίστοιχο μέρος της μελέτης που αφορούσε την αξιολόγηση της ικανότητας της περιστροφικής θρομβοελαστομετρίας στην πρόβλεψη θρομβοεμβολικών επιπλοκών συμπεριλήφθηκαν 307 ασθενείς με κατάγματα ισχίου που υποβλήθηκαν σε χειρουργική επέμβαση. Καταγράφηκαν τα δημογραφικά δεδομένα των ασθενών αυτών και τα αποτελέσματα των συμβατικών εργαστηριακών εξετάσεων της πήξης, ενώ οι ασθενείς παρακολουθήθηκαν για την ανάπτυξη πνευμονικής εμβολής ή εν τω βάθει φλεβικής θρόμβωσης για χρονικό διάστημα 3 μηνών. Η ανάλυση ROTEM πραγματοποιήθηκε προεγχειρητικά και την 2η μετεγχειρητική ημέρα. Τα δημογραφικά δεδομένα, οι συμβατικές εργαστηριακές εξετάσεις της πήξης καθώς και οι παράμετροι της δοκιμασίας ROTEM συγκρίθηκαν μεταξύ των ασθενών με ή χωρίς θρομβοεμβολικές επιπλοκές. Για το αντίστοιχο μέρος της μελέτης που αφορούσε την αξιολόγηση της ικανότητας της περιστροφικής θρομβοελαστομετρίας να ανιχνεύσει ασθενείς υψηλού κινδύνου για μείζονα αιμορραγία και αυξημένες ανάγκες μετάγγισης μετά από επεμβάσεις αποκατάστασης καταγμάτων ισχίου πραγματοποιήθηκε μια προοπτική μελέτη παρατήρησης σε 221 ασθενείς. Η δοκιμασία ROTEM πραγματοποιήθηκε προεγχειρητικά και αμέσως μετεγχειρητικά. Για το αντίστοιχο μέρος της μελέτης που αφορούσε την αξιολόγηση της ικανότητας της περιστροφικής θρομβοελαστομετρίας να ανιχνεύσει ασθενείς υψηλού κινδύνου για μετάγγιση και αυξημένη αιμορραγία μετά από ολικές αρθροπλαστικές ισχίου και γόνατος διενεργήθηκε μια προοπτική μελέτη παρατήρησης στην οποία συμπεριελήφθησαν 206 ασθενείς που υποβλήθηκαν σε ολική αρθροπλαστική ισχίου η γόνατος. Το πηκτικό προφίλ των ασθενών εκτιμήθηκε μέσω της δοκιμασίας ROTEM και των συμβατικών δοκιμασιών της πήξης προεγχειρητικά, και άμεσα μετεγχειρητικά. Η βαρύτητα της αιμορραγίας αξιολογήθηκε μέσω της καταμέτρησης του όγκου απώλειας αίματος, του αριθμό των μεταγγιζόμενων μονάδων ΣΕΑ, και της μετεγχειρητικής πτώσης της αιμοσφαιρίνης. Αποτελέσματα: Στο αντίστοιχο μέρος της μελέτης που αφορούσε την αξιολόγηση των μεταβολών στο αιμοστατικό προφίλ των ασθενών με κατάγματα ισχίου οι συμβατικές δοκιμασίες της πήξης ήταν παρόμοιες για τις 2 ομάδες (υγιείς μάρτυρες και ασθενείς με κάταγμα ισχίου). Ωστόσο, αρκετές παράμετροι της δοκιμασίας ROTEM, συμπεριλαμβανομένων των EXTEM MCF (p < 0.001), ΕΧΤΕΜ γωνία άλφα (p < 0.001), INTEM MCF (p <0.001), INTEM A10 (p < 0.001) και ΙΝΤΕΜ γωνία άλφα (p < 0.001) διέφεραν σημαντικά, υποδεικνύοντας αυξημένη δραστηριότητα της πήξης μετά από κάταγμα ισχίου.
