Vaccination anti-papillomavirus : les facteurs influençant les parents de collégiens dans le choix de la vaccination de leurs enfants : étude prospective menée entre octobre et décembre 2021 dans le département du Calvados

Introduction: In France, 6.300 new cases of genital, anal and ENT cancers attributable to HPV are recorded each year. Vaccination is the only effective prevention against the transmission of this highly contagious STI. However, vaccination coverage is too low to provide herd immunity. The aim of this study was to assess the position of parents, who are key decision-makers, regarding HPV vaccination of their child. Material and method: We conducted a prospective study between October 13th and December 31st, 2021 in 8 volunteer middle schools in Calvados. Parents of secondary school students aged 11 to 14 were interviewed through an anonymous statistical survey platform on which the survey was available. Results: Among the 1.154 parents who participated in the study, 211 (18.3%) claimed to have vaccinated all of their siblings, 225 (19.5%) had only some of them vaccinated, and 718 (62.2%) had not made the decision to have their child(ren) vaccinated. Parents who had vaccinated their child(ren) had significantly better knowledge of HPV. Health professionals (71.1%) were cited by parents as the main source of information, followed by the media (32.8%). Among health professionals, GPs were cited as the main players in recommending vaccination by 76% of parents. The more parents are informed, the more vaccination coverage increases. Conclusion: Information, especially that provided by health professionals, is the key to increasing vaccination coverage. Health professionals occupy a central part in the decision to vaccinate, and must transmit a primary prevention message, that is convincing and reassuring. Furthermore, vaccination in schools appears to be one of the levers for increasing vaccination coverage.Introduction : en France, 6 300 nouveaux cas de cancers de la sphère génitale, anale et ORL attribuables aux HPV sont recensés par an. La vaccination est la seule prévention efficace contre la transmission de cette IST très contagieuse. Or, la couverture vaccinale est trop faible pour permettre une immunité de groupe. L’objectif de cette étude était d’apprécier le positionnement des parents, acteurs décisionnels fondamentaux, concernant la vaccination anti-papillomavirus de leur enfant. Matériel et méthode : nous avons mené une étude prospective entre le 13 octobre et le 31 décembre 2021 dans 8 collèges volontaires du Calvados. Les parents de collégiens âgés de 11 à 14 ans ont été interrogés via une plateforme d’enquêtes statistiques anonyme sur laquelle notre questionnaire était accessible. Résultats : parmi les 1 154 parents participant à l’étude, 211 (18,3%) ont affirmé avoir vacciné la totalité de leur fratrie, 225 (19,5%) n’en ont fait vacciner qu’une partie, 718 (62,2%) n’avaient pas pris la décision de vacciner leur(s) enfant(s). Les parents qui avaient vacciné leur(s) enfant(s) avaient des connaissances significativement meilleures sur les HPV. Les professionnels de santé (71,1%) ont été cités par les parents comme principale source d’information, suivi par les médias (32,8%). Parmi les professionnels de santé, les médecins traitants ont été cités comme principaux acteurs de la recommandation vaccinale par 76% des parents. Plus les parents sont informés, plus la couverture vaccinale s’accroît. Conclusion : l’information, et notamment celle délivrée par les professionnels de santé, est la clé de l’augmentation de la couverture vaccinale. Les professionnels de santé occupent une place centrale dans la décision de vaccination, et doivent transmettre un message de prévention primaire, convainquant et rassurant. De plus, la vaccination en milieu scolaire apparaît comme l’un des leviers pour augmenter la couverture vaccinale
