Most appropriate spirometric reference values for populations of European origin

Introdução: A adequabilidade dos valores de referência espirométricos é imprescindível para o diagnóstico e seguimento das doenças respiratórias. Atualmente existem mais de 70 estudos publicados sobre valores de referência. A American Toracic Society e a European Respiratory Society (ATS/ERS) recomendam que os laboratórios selecionem os valores de referência mais adequados às suas populações e, quando indicado, desenvolvam equações de referência próprias. A presente revisão teve como objetivo perceber quais as equações de referência mais apropriadas às populações de origem europeia. Metodologia: Realizou-se uma pesquisa na Natural Library of Medicine PubMed – Medline. Os filtros aplicados foram língua inglesa e espécie humana. Foram incluídos artigos originais encontrados entre janeiro de 2007 e dezembro de 2017. Foram definidos como critérios de inclusão: obtenção de valores de referência para os parâmetros espirométricos tais como forced vital capacity (FVC) e forced expiratory volume in one second (FEV1) numa população adulta, saudável de origem europeia; e que os resultados dos estudos apresentassem a média das diferenças entre os valores medidos e os valores de referência (resíduos), em forma numérica. Na pesquisa foram encontrados 146 artigos, sendo que apenas cinco cumpriam os critérios de inclusão definidos nesta revisão. Resultados: Os valores de referência provenientes de estudos locais (mesma população ou populações geograficamente próximas) foram os mais próximos dos valores medidos. Por outro lado, os valores de referência da European Coal and Steel Community (ECSC) foram os mais distantes dos valores medidos para a FVC e FEV1, na maioria dos estudos. Conclusões: Nesta revisão, os valores de referência mais adequados foram os obtidos em populações locais ou geograficamente próximas e os da ECSC os mais distantes. Para seleção dos valores de referência deve-se idealmente ter em conta os desvios que apresentam em relação à população em estudo.ABSTRACT - Introduction: The suitability of spirometric reference values is essential for the diagnosis and follow-up of respiratory diseases. There are currently more than 70 published studies on reference values. The American Thoracic Society and the European Respiratory Society (ATS/ERS) recommend that laboratories select the most appropriate reference values for their populations and, when indicated, develop their own reference equations. The purpose of this review was to understand which equations of reference are most appropriate for populations of European origin. Methodology: Research was done at the Natural Library of Medicine PubMed - Medline. The applied filters were the English language and the human species. Original articles were found between January 2007 and December 2017. Inclusion criteria were defined as: reference values for spirometric parameters such as forced vital capacity (FVC) and forced expiratory volume in the first second (FEV1) in a healthy adult population of European origin; and the results of the studies presented the mean differences between the measured values and the reference values (residues), in numerical form. A total of 146 articles were found, out of which only five met the inclusion criteria defined in this review. Results: Reference values from local studies (the same population or geographically close populations) were the closest to the measured values. On the other hand, the European Coal and Steel Community (ECSC) reference values were the furthest from the values measured for FVC and FEV1 in most studies. Conclusions: In this review, the most adequate reference values were those obtained in local or geographically close populations and those of the ECSC were the most distant. In order to select the reference values, one should ideally take into account the deviations they present in relation to the study population.
