Effects of transcranial direct current stimulation (tdcs) on anti-inflammatory Cytokines : a systematic review

Citocinas são pequenas proteínas expressas na superfície da célula, possuem ações pró ou anti-inflamatórias, de acordo com o ambiente onde estão inseridas. Citocinas anti-inflamatórias limitam a resposta inflamatória atuando como um regulador de feedback negativo, mantendo a resposta imunológica equilibrada. A Estimulação Transcraniana por Corrente Contínua (ETCC) é uma técnica neuromoduladora que atua no sistema nervoso central (SNC) utilizando uma corrente contínua de baixa intensidade. Trata-se de uma abordagem terapêutica não farmacológica promissora no tratamento de dor crônica e distúrbios neuropsiquiátricos. Estudos sugerem a ação antiinflamatória da ETCC por meio da modulação dos níveis de citocinas, mas pouco se sabe sobre os mecanismos dessa terapia. Assim, o objetivo deste trabalho é identificar o efeito da ETCC na modulação dos níveis de interleucinas anti-inflamatórias no SNC de animais, por meio de uma revisão sistemática da literatura. Foram utilizadas publicações científicas disponíveis durante o período de busca, de janeiro de 2020 a janeiro de 2021 nos bancos de dados PubMed, Scielo e LILACS. Foram usados os seguintes descritores (MeSH): ‘Transcranial Direct Current Stimulation’; ‘tDCS’, ‘interleukin 4’, ‘IL 4’, ‘IL-4’; ‘interleukin 10’, ‘IL 10’, ‘IL-10’; ‘interleukin 13’, ‘IL 13’, ‘IL-13’; ‘Transforming Growth Factor beta 1’, ‘TGF-beta1’, ‘TGF-beta-1’; ‘interleukin 11’, ‘IL 11’, ‘IL-11’; ‘interleikin 1 Receptor Antagonist Protein’, ‘IL-1Ra’. Foram identificados 141 estudos, no entanto apenas 6 atingiram os critérios de inclusão final. Foi observado que a ETCC tem capacidade de modular as citocinas anti-inflamatórias IL-10 e IL-4. Além disto, a esta eletroterapia tem mostrado ser efetiva na promoção de analgesia. No entanto, mais estudos devem ser conduzidos para elucidar os mecanismos envolvidos.Cytokines are small proteins expressed on the cell surface, they have pro or anti-inflammatory actions, depending on the environment where they are inserted. It is a promising non-pharmacological therapeutic approach in the treatment of chronic pain and neuropsychiatric disorders. Studies suggest the anti-inflammatory action of tDCS through modulation of cytokine levels, but little is known about the mechanisms of this therapy. Thus, the objective of this work is to identify the effect of tDCS on the modulation of anti-inflammatory interleukin levels in the CNS of animals, through a systematic review of the literature. Scientific publications available during the search period, from January 2020 to January 2021 in the PubMed, Scielo and LILACS databases were used. The following descriptors (MeSH) were used: 'Transcranial Direct Current Stimulation'; 'tDCS', 'interleukin 4', 'IL 4', 'IL-4'; 'interleukin 10', 'IL 10', 'IL-10'; 'interleukin 13', 'IL 13', 'IL-13'; 'Transforming Growth Factor beta 1', 'TGF-beta1', 'TGF-beta-1'; 'interleukin 11', 'IL 11', 'IL-11'; ‘interleikin 1 Receptor Antagonist Protein’, ‘IL-1Ra’. 141 studies were identified, however only 6 met the final inclusion criteria. It was observed that tDCS has the ability to modulate the anti-inflammatory cytokines IL-10 and IL-4. In addition, this electrotherapy has been shown to be effective in promoting analgesia. However, further studies should be conducted to elucidate the mechanisms involved
