Investigation of the response relationships of egfr mutations to chemotherapeutic drugs and the effects of these mutations on the expressions of p53, pten, trail receptor, fas receptor, survivin, bax and bcl-2 on patients diagnosed with lung cancer.

Akciğer kanserleri, Dünya genelinde kanser nedenli ölüm sebepleri arasında ilk sırada yer almaktadır. Prostat ve meme kanserlerinden sonra akciğer kanserleri üçüncü sırada görülmektedir. Akciğer kanserlerinde Epidermal Büyüme Faktör Reseptöründe (EGFR) meydana gelen mutasyonlar özellikle tirozin kinaz bölgesi mutasyonlarıdır. Küçük Hücre Dışı Akciğer Kanseri (KHDAK) tümörleri özellikle Asyalı, beyaz ve sigara içmeyen bayanlarda daha sık görülmektedir. Bu çalışma da KHDAK hastalarında EGFR- Kirsten Ras Sarcoma (KRAS) mutasyon profilleri, erlotinib ilişkisi ve p53, pten, trail reseptör, fas reseptör, survivin, bax ve bcl-2 ifadeleri araştırılmıştır. EGFR mutasyonuna sahip 97 hastanın 39’unda delE746-A750 (%36.4), 29’unda L861Q (%18.5), 10’unda L747-T751 ins P (%12.3), 10’unda L858R (%9.8), 9’unda G719A (%9.8), 5’inde del E746-T751 ins P, 3’ünde T790M (%3.6), 3’ünde G719S (%3.6), 3’ünde del E746-A750 ins I, 3’ünde L747-P753 ins S, 1’inde delE746-A750 IP (%1.2), 1’inde G719C (%1.2), 1’inde L747-T751 ins Q tespit edilmiştir. KRAS mutasyonuna sahip 75 hastanın 32’sinde G12V (%6.6),10’unda G12S (%22.3), 8’inde G12C (%17.8), 6’sında G12D (%13.3), 18’inde Q61R (%15.5), 15’inde Q61H (%22.3), 1’inde G12A (%2.2), 1’inde G13D tespit edilmiştir EGFR ve KRAS mutasyonun ikisini birden sahip 20 hasta (%6.6) tespit edilmiştir. EGFR mutasyonları demografik olarak kadın (%48 kadın, %28 erkek), sigara içmeyen (sigara içmeyen %49, sigara içen %26) ve adenokarsinomalı (adenokarsima %37.8, skuamöz %26.8) hastalarda istatistiksel olarak anlamlı bulunmuştur. Bu hastalarda EGFR mutasyonu daha sık tespit edilmiştir. Bu hastalardan sağkalım oranı EGFR-TKI tedavisi alan hastaların almayanlara göre tüm sağkalımı oranı istatistiksel olarak anlamlıdır (146±22, 84±12(hafta) p:0.020). KHDAK hastaların tümör dokularında EGFR ve KRAS mutasyonları Bcl-2 ve survivin expresyonlarını artırırken Bax, p53, PTEN, FADD ve TNFR ekspresyonlarını azalttığı saptanmıştır. EGFR ve KRAS mutasyonları KHDAK hastalarında antiapoptotik yolakları indüklerken, apoptotik yolakları baskılamaktadır. Sonuç olarak KRAS mutasyonları KHDAK hastaları için yeni bir prognostik faktör olabilir. Lung cancer is the leading cause of cancer death in the World. Breast and prostate cancers are more common than lung cancer. Mutations in the EGFR gene are critical determinants of treatment with Epidermal Growth Factor Receptor (EGFR) tyrosine kinase inhibitors (TKIs) for non-small cell lung cancer (NSCLC) patients. The frequency of EGFR mutations is reported high among white Asian, female and non-smokers. DNA was isolated from the formalin-fixed, paraffin- embedded 300 Turkish NSCLC patients and their EGFR mutation status was examined by pyrosequencing. EGFR mutations were detected in 97 of the 300 patients (36%); 39 patients with delE746-A750, 29 patients with L861Q, 10 patients with L747-T751 ins P, 10 patients with L858R, 9 patients with G719A, 3 patients withT790M, 3 patients with G719S, 5 patients with delE746-T751 ins I, 1 patients with delE746-A750 IP, 1 patients with delL747-P753 S and 1 patients with G719C. Kirsten Ras Sarcoma (KRAS) mutations were detected in 75 of the 300 patients (20.8%); 10 patients with G12S, 8 patients with G12C, 6 patients with G12D, 20 patients with G12V, 15 patients with Q61H, 18 patients with Q61R and 1 patients with G12A. EGFR and KRAS mutations both detected in 20 patients (%6.6).
