The effects of risperidone on behaviors seen in children with pervasive developmental disorder and mental retardation in an educational setting

Amaç: Yaygın gelişimsel bozukluk (YGB) ve mental retardasyon (MR) tanısı alan çocukların psikiyatrik tedavisinde aşırı sinirlilik, saldırganlık, şiddet ve kurallara uymama gibi özgül olmayan belirtilerin düzelmesi yanında olguların sosyal uyumunu düzeltme ve özel eğitim alabilir hale gelebilmeleri için nöroleptikleri kullanmak gerekebilir. Bu çalışmada amaç, DSM-IV tanı ölçütlerine göre YGB ve MR tanısı konan 8 çocukta atipik nöroleptiklerden risperidonun eğitim ortamındaki olumsuz davranışlar üzerine olan etkilerini araştırmaktır. Yöntem: Yaşları 7-17 arasında olan 8 olgunun değerlendirmesi ve takibinde Öğretmen Bilgi Formu, Conner’s Dikkat Eksikliği Aşırı Hareketlilik Öğretmen Değerlendirme Formu (Conner’s) ile Genel Klinik İzlenim Ölçeği (GKİ) kullanıldı. Olgulardan ikisine birincil tanı olarak otistik bozukluk, diğer ikisine Asperger bozukluğu, kalan dördüne de mental retardasyon tanısı kondu. Risperidonun günlük kullanım dozu 1-4 mg olarak belirlendi. Tedaviye başladıktan 8 hafta sonra çalışmanın başında yapılan değerlendirmeleri tekrarlandı. Bulgular: Tedavinin başlamasından 8 hafta sonraki takipte Öğretmen Bilgi Formu’nda sakinleşme, verilen komutlara uymada artma, dinleyebilme ve sosyal ilişkide göreceli düzelmelerin olduğu gözlendi. Conner’s ve GKİ ölçek değerlerindeki istatistiksel açıdan anlamlı değişiklikler bu gözlemleri destekler nitelikteydi. (Conner’s için p=0.03; GKİ için p= 0.007) Tartışma: Dikkati yoğunlaştırma sorunları ve artmış hareketlilik belirtilerindeki olumlu değişimlerle ile birlikte eğitim becerilerinin artması ve daha az yan etki nedeniyle atipik nöroleptiklerden risperidon bu hasta grubundaki davranış sorunlarında kullanılabilecek farmakolojik seçeneklerden biri olabilir.Objective: The presence of nonspecific behavioral problems such as agression, violence, and inability to attend to the rules seen in pervasive developmental disorders (PDD) and mental retardation (MR) make it necessary to use neuroleptics in these children and adolescents to decrease the behavioral symptoms to achieve an increased ability in the regulation of social competence and better attending to special education. in this study we aimed to find out the effects of an atypical neuroleptic, risperidone, on behaviour modification in an educational setting. Eigth children having the DSM-IV diagnosis of PDD and MR were studied. Method: Teacher's Report Form, Conner's Attention Deficit Hyperactivity Disorder Teacher Scale (Conner's), and Clinical Global Impression (CGI) Scale were used in evaluation and follow up of 8 children, between the age ranges of 7-17. Two of them had autistic disorder, the other two had Asperger's disorder, and the rest had MR as a primary diagnosis according to DSM-IV criteria. Risperidone was in doses of 1-4 mg/day and subjects were re-evaluated after 8 weeks of treatment period. Results: There was a considerable increase in ability to respond to limit setting, to follow the instructions, and better social competence in Teacher's Report Forms after 8 vveeks of treatment period. Statistically significant changes in Conner's and CGI supported this observation (for Conner's, p= 0.03; for CGI, p= 0.007). Conclusions: Besides the positive effects on attention, concen tration and hyperactivity, better utilization of learning skills and well tolerated ,side effects could make atypical neuroleptic risperidone a pharmacologic alternative for treating behavioral symptoms in these particular patient groups
