ASUHAN KEPERAWATAN PADA NY. D DENGAN CHRONIC KIDNEY DISEASE (CKD) DI RUANG RAWAT INAP H RS WAVA HUSADA

Background: Chronic Kidney Disease (CKD) or Chronic Kidney Failure is a condition in which the kidneys experience structural abnormalities or functional disorders that have lasted more than 3 months. In this final scientific paper, the case of Mrs. D with a diagnosis of Chronic Kidney Disease (CKD) which is suspected to be due to a history of hypertension that the patient had previously suffered. For this reason, this final scientific paper aims to describe the process of Nursing Care for Ny. D with Chronic Kidney Disease (CKD) in Inpatient H Hospital Wava Husada Kepanjen. Methods: The research method used in the preparation of this final scientific paper uses case study reports. The author follows the scientific method according to the rules of the nursing process which includes: assessment, data analysis, nursing intervention, implementation, and evaluation. Result: After nursing care for 3 days in a row, the authors raised 4 nursing problems, namely hypervolemia, ineffective peripheral perfusion, acute pain, and non-compliance. Of the 4 nursing problems, there are 2 nursing diagnoses that have not been resolved. Discussion: Of the four nursing problems, hypervolemia is a priority diagnosis. The interventions applied to the patients were management of hypervolemia, diet education, pain management, and support for adherence to treatment programs. The evaluation obtained was that fluid output was more than fluid intake (intake of 400 ml/day and output of 650 ml/day), pale skin color had decreased considerably, systolic and diastolic blood pressure had improved sufficiently to 110/70 mmHg, pain on a scale of 3 (mild pain), the patient adhere to care and treatment
