PENGELOLAAN KONTROL GULA DARAH DAN ULKUS DIABETIKUM PADA Tn.N Di Ruang Serly – Rumah Sakit Muhammadiyah Malang

Background : Diabetes Mellitus (DM) is a serious chronic disease caused by the pancreas not being able to produce enough insulin or when the body cannot effectively use the insulin produced by the pancreas, insulin is a hormone that functions to regulate blood glucose in the body. In this scientific paper, the case of Mr. N with a diagnosis of Diabetes mellitus with diabetic ulcers is presented. This scientific paper aims to study and understand about nursing care for Mr. N with diabetes mellitus with diabetic ulcers in the Serly room of the Muhammadiyah Malang Hospital. Methods : This final scientific paper for nurses uses a research design with a case study approach to explore the problem of nursing care in patients with diabetes mellitus accompanied by ulcers in the Serly room of Muhammadiyah Hospital Malang. In the assessment process the author uses various data collection techniques such as anamnesis, observation, study documentation and physical examination to collect various data from the patient, namely Mr. N to complete the data, the author also conducts interviews with the family of Mr. N. Results : After providing nursing care for 3 consecutive days, the authors raised 4 priority nursing diagnoses, namely instability of blood glucose levels, impaired skin integrity, acute pain and impaired physical mobility. The patient came with complaints of pain and swelling in his left leg. During the treatment, insulin therapy was given and debridement was performed. On the fourth day of treatment, Mr.N's blood glucose levels were normal so he was allowed to go home Discussion : Of the 4 nursing problems, instability of blood glucose levels is a priority diagnosis. Interventions that have been applied to patients are hyperglycemic management and wound care. The implementation is in accordance with the nursing intervention plan. The evaluation obtained was the patient's blood glucose level of 110 mg/dL, the color of the wound was reddish, there was no pus, the depth of the wound was ±1 cm, the width of the wound was ±7 cm.
